This position offers an exciting opportunity to be part of Community Connections Program, part of the Community Mental Health Service of St. Michael's Hospital; a teaching hospital in downtown Toronto. The successful candidate will be part of an interdisciplinary team (psychiatrist, occupational therapist, social worker, registered nurse) that provides outreach, comprehensive psychiatric and psychosocial interventions through intensive case management model for individual adults from the ages of 18 to 65, who have been experiencing symptoms of psychotic disorders. The team implements best practices in relation to help individuals maintain a connection with their community.

DUTIES & RESPONSIBILITIES:

  • Provision of Occupational Therapy to promote, maintain or restore health and wellbeing through engagement in meaningful occupations in the areas of self-care, productivity and leisure. Occupational therapy services will include assessment, treatment, consultation, discharge planning and education to patients, family members/caregivers, external agencies and SMH staff.
  • Utilizes assertive outreach strategies, to locate, engage and maintain clients in a helping relationship. Delivers service and supports in the client's environment of choice in the community (e.g. home or other community setting). Adjusts frequency and length of outreach visits based on client needs and goals and team consultations
  • Able to work effectively as a member of an interprofessional team, collaborating with other disciplines to provide comprehensive patient care
  • Contributes to the academic mandate of the organization as an educator, resource or support for staff, students and members of the academic and professional community
  • Identifies and addresses educational needs for patients and their families
  • Demonstrates commitment to ongoing professional development in order to continue to provide optimal patient care
  • Is committed to, and participates in, ongoing quality improvement in the organization
  • Provides patient care in accordance with all professional and organizational standards
QUALIFICATIONS:
  • Must have a Master's degree (or equivalent) in Occupational Therapy
  • Must be currently licensed by and in good standing with the College of Occupational Therapists of Ontario (COTO) for practice in all applicable clinical areas.
  • Current CPR certification
  • Excellent communication, critical thinking and interpersonal skills
  • Proven flexibility and adaptability in a fast-paced environment
  • Excellent time management, organizational and prioritization skills as relates to patient care
  • Demonstrated commitment to and involvement with own profession (e.g. membership with professional association, etc.)
  • Knowledge of psychosocial rehabilitation and recovery as it applies to community mental health settings
  • Knowledge of case management models and case management functions
  • Knowledge of medical, pharmacological and other treatment strategies for individuals with psychosis and substance abuse disorders

Must be willing to work evenings and/or weekends as directed by the Program.
