Under the general direction of the hospital's Long Term Care Director, The Registered Dietitian will create nutrition plans to improve the health and physical conditions of patients. This role will have a substantial impact to the hospital's Dietary/nutrition services and play a key role for the hospital by observing the dietary needs and providing answers to combat malnourishment for patients admitted within the Long Term Care facility. Some of these operational impacts include but are not limited to serving as the point of contact for analyzing patients' health status, goals and dietary restrictions, tracking clients' dietary intake and progress and educating patients on how to maintain a balanced diet.

ESSENTIAL FUNCTIONS:

Must be able to work independently and under sustained pressure and during stressful situations. This role will perform responsible and moderately complex mid-level duties by overseeing clinical nutrition assessment, follow up, and nutrition counseling for new admissions, weight variances, pressure injuries, tube feedings, hemodialysis, impaired chewing/swallowing ability, etc. Make culturally and age-appropriate recommendations for nutritional interventions and provide medical nutrition therapy per standards of practice. This position must offer great technical skills and customer service to the hospital's Long Term Care Residents and acts as a liaison between other departments and the care team. This role will provide quality and regulatory assurance review of Food/Nutrition Services sanitation, food storage, preparation, and dining services. The Registered Dietitian will works closely with patients and other medical professionals as part of the interdisciplinary team (IDT) to set and achieve nutritional goals. This role will serve as one of the primary educators to teach Nutrition education and in-service training for food/nutrition service staff, nutritional process and best practice standards for enhancing a positive dietary results.

DUTIES AND RESPONSIBILITIES: Administers practices and tasks relating to all phases of Nutritional activities by performing the following duties:

  1. Comprehensive nutrition assessment of all patients must be within the nutrition parameters of the MDS (Multiple Data Sheet) guidelines for the CMS (Center for Medicaid/Medicare Services).
  1. Keeps lines of communication open between Medical Staff and Dietary Department. Meets with Medical Advisor to discuss physician's issues and medical/nutrition-related project.
  2. Interprets physician diet orders and modifies diet according to diet patterns and patient preferences
  3. Interviews patients assigned and document pertinent information on the appropriate nutrition documentation forms. Provides scheduled patient follow-ups of planned dietary/nutrition intervention.
  4. Instructs patient(s) (or family if needed), in managing their dietary needs as applicable to their conditions, dietary pattern and eating lifestyle.
  5. Evaluates outcome(s) of dietary intervention and nutritional care planned for each patient.
  6. Provides substitutions if patient is unable to eat the meal provided, in accordance with diet restrictions
  7. Plans between-meal nourishments for patients as necessary
  8. Relays information on assigned patients to food production staff and Dietary Aide. This information is documented on the patient's nutrition profile in the computer
  9. Completes daily calorie counts when ordered or initiated
  10. Completes a nutritional assessment on patients receiving a tube feeding or parenteral nutrition
  11. Computes nutritional value of recipes when necessary
  12. Completes diet histories for nutritional assessment on specific patients
  13. Evaluates appropriateness of diet order for diagnosis and recommends diet order changes as indicated
  14. Maintains updated and necessary patient education materials and nutrition references and textbooks
  15. Plans, coordinates and implements the department's Quality Assurance Program
  16. Participates in the development of policies and programs directed toward patient and family education
  17. Directs Diet Aides in daily duties and offers in-service education
  18. Participates in patient care plan meetings, support groups as requested or deemed necessary, and Dietary Department management meetings
  19. Participates in department and implementation educational programs and meets the mandatory in-service requirement in utilities management, infection control, safety and blood-borne pathogens

SKILLS AND ABILITIES REQUIRED:
  1. To perform this job successfully, an individual should have knowledge of comprehensive assessment skills and full understanding of the Center for Medicare/Medicaid Service (CMS) guidelines from a suitable clinical experience.
  2. Good understanding and application of the whole nutrition care process (NCP).
  3. Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form.
  4. Ability to deal with problems involving several concrete variables in standardized situations.
  5. Should have a working knowledge of EMR system and word processing software.
EDUCATION AND EXPERIENCE REQUIRED:
  1. Current registration with the Commission of Dietetic Registration (CDR) of the American Academy of Nutrition and Dietetics (A.N.D.).
  2. Current licensure by the Nevada State Board of Health Division.
  3. Must possess a current Health Card.
