The Pharmacy Care Coordinator provides administrative support for the pharmacist, including but not limited to patient check-in calls, provider recommendation follow-up, and tracking of patient outcomes.

Location: This role is Remote Nationwide

Schedule: Monday - Friday, 8:30am - 5pm MST

You will enjoy the flexibility to telecommute* from anywhere within the U.S. as you take on some tough challenges.

Primary Responsibilities:

  • Outreach patients to obtain medication history or to follow-up on medication changes as directed by pharmacist
  • Triage patients needing pharmacist consultation
  • Provide support and guidance to patients on medication adherence
  • Exercise positive customer service and communication skills with patients and other healthcare professionals
  • Assist in coordinating care with other healthcare professionals
  • Documents pharmacist intervention outcomes
  • Follows up on pharmacist recommendations with providers via fax and telephone
  • Performs all other related duties as assigned

Knowledge/Skills/Abilities:

  • Excellent communication and customer service skills
  • Self-motivated with critical thinking skills
  • Works well with others and thrives in an interdisciplinary environment
  • Knowledge of pharmacy and medical terminology
  • Knowledge of common medication dosages and brand/generic names
  • Strong computer skills, including Microsoft Office Suite (Excel)

Required Qualifications:

  • High School Diploma / GED (or higher)
  • Active and unrestricted Pharmacy Technician license in the state of Colorado OR National Pharmacy Technician Certification OR ability to obtain within 90 days of employment
  • 1+ years of experience in managed health care or medication therapy management

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
