As a Per Diem Home Health Physical Therapist with us, every visit is an opportunity to make a genuine impact. From assessing patients' needs to documenting their progress with warmth and professionalism, you'll lead the charge in enhancing their quality of life. Responsibilities:
Immerse yourself in patient care by assessing their condition during each visit and meticulously recording your observations.
Embrace the art of evaluation, from vital signs to daily activities, ensuring our patients receive personalized, top-notch care.
Utilize your expertise to assess and enhance therapeutic, rehab, and functional statuses, bringing smiles and progress along the way.
Be a hero by recommending devices or tools that could add extra joy to our patients' lives.
Provide the attentive ear our patients need by listening to their concerns and fulfilling their requests with grace.
Collaborate seamlessly with our multidisciplinary team to craft tailored care plans for each individual, including Registered Nurses, Occupational Therapists, Physicians, and more.
Educate patients and their families on effective home health care strategies, empowering them with knowledge and confidence.

Requirements:

Embrace the adventure of exploring Tulare and Kings County's dynamic neighborhoods as you visit multiple patients each day.
Flaunt your State Physical Therapy license from the Physical Therapy Board of California as your badge of honor.
While experience in in-home or Hospice settings is advantageous, your passion and dedication are what truly count.
Communicate effectively and manage time efficiently, balancing efficiency with a personal touch.
Get ready to hit the road with a valid driver's license, as you embark on a journey of healing and connection.
Keep your CPR certification and TB clearance up to date to ensure you're always prepared to spring into action.

Required Experience:
Let's kickstart this adventure together with at least 1 year of experience in Physical Therapy.

Required License or Certification:
Unlock your dreams with a valid Physical Therapy license.
