General Purpose of Job:

The NICU Discharge Coordinator oversees a specific area of service and supports the day-to-day operation of Driscoll Children's Hospital vision of excellent and integrated patient care delivery; engenders and participates in collaborative decision-making; ensures compliance with establish standards; and contributes to creating an environment that fosters patient advocacy and competence.

• Maintains utmost level of confidentiality at all times.

• Adheres to hospital policies and procedures.

• Demonstrates business practices and personal actions that are ethical and adhere to corporate compliance and integrity guidelines.

• Fulfills all duties and expectations of staff RN if appropriate.

• Coordinates and oversees day-to-day operations within an area of service.

• Participates in process improvement activities that facilitate continuous quality improvement as measured by patient and or customer service outcomes.

• Promotes family centered concept by functioning as a liaison with the family, guest relations, and appropriate personnel.

• Functions as a liaison with all service physicians and ancillary services on a day-to-day basis.

• Follows established hospital guidelines required to determine deployment of resources.

• Identifies performance issues and participates in the performance evaluation and competency assessment processes as directed by manager/director.

• Supports best practice through coordination with available resources.

• Routinely assumes role of charge nurse and/or staff nurse.

Prioritizes, evaluates, and communicates to adjust to rapidly changing condition by using critical thinking skills.

• Assess patients and families for post discharge needs; inform patients and families on discharge planning options based on diagnoses, prognoses, resources and preferences related to home care services.

• Coordinate and facilitate timely implementation of discharge plans for assigned patients and arrange follow-up care as appropriate.

• Maintain effective communication with physicians, nursing staff, patients, families and others related to discharge planning; coordinate with social service personnel to provide services needed.

• Document findings, discharge plans and actions taken on medical record according to departmental guidelines; prepare reports and maintain records as required.

• Acts as an educational resource and provide consultation to hospital medical personnel regarding discharge planning process and applicable federal, state, and local regulation; identify benefits, implications and limitations of home care as appropriate.

• Participate in multidisciplinary team meetings regarding the planning and implementation of patient care; facilitate communication and problem solving related to discharge planning

• Perform other related duties incidental to the work described.

CERTIFICATES,LICENSES,REGISTRATIONS:

• Licensed in State of Texas as an RN

• PALS Certified

• BLS current at hire

• Certification in specialty preferred
