Shift Details: 9a-6p - Mon-Fri, Saturdays 9a-4p.
Contract Length: ongoing until a perm is hired
Schedule: Ideally, 5 days per week, with one Saturday every other week. May consider part-time candidates
Support Staff: Each location has two DA's scheduled per provider.
Department Description: 3 dental chairs, average 17-20 patients per day, all ages. Medicaid heavy population
Specific travel client will pay for: Travel and Lodging
Credentialing Timeframe: 2-4 weeks

Requirements:
  • Name Clear Required - name and NPI
  • EMR: Denticon
  • Bread and Butter Dentistry - Must be comfortable with extractions, dentures, crowns & general dentistry (including cleanings....no hygienists for the most part).
  • Would prefer implant restoration.
  • Endo preferred, but can be flexible
  • Will consider new grads
  • License Required: Active IL license without restrictions - may consider licensing candidates.
  • Certifications Required: BLS, DEA, CSR - 80% of patients are Medicaid, need to be enrolled, will help enroll if needed
