Job Title: LVN - Triage

Salary: $30/hr + DOE

Location: Tustin, CA

Openings: 1) Position Available

***This position will require working every Saturday from 9am - 3pm. *** Time off will be given during the week to compensate.

Summary

The LVN Triage Nurse is responsible for providing triage (sorting/prioritizing patients) over the telephone to assist in assessing patient care with medical provider.

JOB RESPONSIBILITIES:

• Performs short evaluation of the patient situation to determine their medical needs and directs them to primary, support and/or referral services as needed.
• Communicates with patients over the telephone to ensure follow-up care has been issued and/or completed.
• Documents medical status information as needed.
• Determines urgency of seeing a patient based on brief assessment and on familiarity with a patient's condition and history. May use computerized medical information database and confers with medical provider when needed.
• Sets up appointments for patients to see a medical provider or arranges communication with a contact center representative to assist patient.
• Coordinates transitions of care for patients as needed.
• Serves as liaison between clinic and Health Department regarding reportable illnesses.
• Other duties as required by position.

• High school diploma or equivalent required.
• Graduation from an approved vocational nursing program required.
• Current, active state vocational nursing license required.
• Bilingual: Spanish required.
• At least one year of LVN experience in a high paced clinical setting preferred.
• Qualified candidates must have strong interpersonal skills, must be able to maintain effective working relationships with co-workers, and effectively communicate and interact with diverse patient populations.

• Knowledge of medical terminology, anatomy, physiology and pathophysiology.
• Familiarity with health care systems, regulations, policies and functions, and documentation standards.
• Knowledge of infectious disease management and control and safety standards.
• Skill in initiating appropriate emergency procedures.
• Ability to perform mathematical calculations for drug doses.
• Ability to handle effectively multiple tasks simultaneously.
