The Physical Therapist will provide outpatient rehabilitation services in an in-home setting for older adults, either in their private residence or an independent living facility. Responsibilities include evaluating and treating patients, developing and implementing physical therapy programs, and making necessary referrals to other disciplines. The role also involves educating and training patients and caregivers to promote independence and productivity. This position offers a flexible schedule and work environment.

Responsibilities:

  1. Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel as per state licensure and professional standards.
  2. Adhere to treatment schedules, ensure timely documentation, and attend relevant meetings.
  3. Participate in patient care conferences and related meetings.
  4. Refer patients to other disciplines and agencies as needed.
  5. Collaborate with the rehabilitation team to select appropriate therapeutic regimens.
  6. Adapt care plans to meet individual patient needs.
  7. Promote a safe environment and address safety hazards.
  8. Order, clean, maintain, and report the need for equipment repairs.
  9. Ensure high-quality, compassionate care for patients and families.
  10. Conduct thorough patient assessments and develop, implement, and document treatment plans. Services may include:
    • Therapeutic exercise (aerobic conditioning, balance training, etc.)
    • Functional training
    • Manual therapy
    • Airway clearance techniques
    • Integumentary repair and protection
    • Use of electrotherapeutic, physical agent, and mechanical modalities
    • Health and wellness education
    • Chronic disease management
    • Falls assessment and interventions
    • Case management in home and community settings
  11. Design, fit, and train patients in the use of adaptive devices.
  12. Select activities suited to the patient’s physical and mental capacities.
  13. Instruct patients in mobility skills and evaluate progress.
  14. Perform routine reexaminations to modify treatment plans.
  15. Integrate health and wellness programming into treatment plans.
  16. Conduct home and community mobility assessments and provide safety recommendations.
  17. Perform other related duties as required.

Requirements:

  • Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
  • Master’s degree in Physical Therapy, or a Master’s degree in a related field with a Bachelor’s degree in Physical Therapy, or a Bachelor’s degree in Physical Therapy plus 5 years of experience.
  • Foreign Physical Therapy degrees must be evaluated as equivalent to a U.S. degree.
  • Valid driver’s license or reliable transportation to patients’ residences.
