Overview
We are seeking a collaborative, outpatient, orthopedic physical therapist to provide pelvic health PT services at our M Health Fairview Rehab Services Elk River & Princeton locations. The Elk River and Princeton M Health Fairview Rehab Services teams partner to provide outpatient, multi-specialty therapy services to the northwest Twin Cities metro area. Preferred candidate will have experience in providing interventions for Women’s, Men’s, and Pediatric pelvic health populations. Where training and experience is limited, M Health Fairview offers mentoring and development opportunities for professional growth.

FTE 1.0, authorized for 80 hours per pay period.

Schedule: Two, 10-hour days in each location (Elk River and Princeton). Schedule flexible but ideally Elk River on Tuesdays and Princton Fridays.

M Health Fairview Rehabilitation offers a broad range of services that serve patients across ten acute-care hospitals, three post-acute settings and 70 outpatient adult and pediatric therapy clinics. Consisting of Physical, Occupational and Speech Therapy as well as Audiology and Cardiac & Pulmonary Rehab, our therapists collaborate with colleagues in all medical settings and offer dozens of specialty programs.
As an academic health system with residency and fellowship programs and a rehab-focused clinical quality team, we have a collaborative culture that is centered on learning with an emphasis on evidence based, patient-centered care. Rehab’s continuing education program offers 40+ continuing education courses per year at no cost to employees and further supports numerous external CE courses each year.

Responsibilities Job Description
The Physical Therapist partners with the patient and their care providers while performing evaluation and re-evaluation. Captures and applies pertinent data, performs examination, and determines treatment diagnosis
Develops an individualized plan of care through collaboration with the patient and family
Analyzes data to identify needs and potential risks, settles prognosis for recovery, and establishes individualized goals
Implements and modifies care plan based on reassessment and patient response
Coordinates care and communicates effectively with interdisciplinary team for all aspects care & establishes appropriate discharge plan
Partners with patients and families to provide skilled intervention
Implements and alters treatment to reflect change in patient status and response to treatment
Assesses patient and/or caregiver learning needs and provides appropriate education
Documentation/Billing
Uses acceptable medical terminology and follows professional practice standards.
Professional Expectations
Prioritizes tasks and patient care for effective time management.
Completes documentation in a timely manner per department standards
Stays in sync with department billing standards
Meets or exceeds departmental standards for productivity
Identifies opportunities for process /quality improvement
Crafting and implementing improvements, and participates in and supports entity/system initiatives
Understands and maintains licensure/certification requirements
Advises team members and/or supervises students as designated
Pursues professional development to support clinical standard processes

Travels to and provides care in multiple settings as assigned

Requires access to reliable transportation to be present at various locations to conduct and meet reasonable business needs
Qualifications
Required License/Certification/Registration
Physical Therapy registration/licensure in the state of Minnesota or temporary license in MN
Current Basic Life Support (BLS) certification
Preferred Education & Experience
Doctorate degree in Physical Therapy
1+ year of physical therapy practice
1+ year experience with pelvic health
