About the Role:

Astrana Health is looking for a LVN - Hospice to work at our Alhambra locations, 1680 S. Garfield Ave Suite 205 Alhambra, CA 91801. You will be required to travel to patient homes, but you will be reimbursed at the IRS mileage rate. This position's schedule is flexible..

While you will not directly supervise anyone when you start, you will have the ability to obtain
supervisory responsibilities as we grow. This position is a great opportunity for a newer LVN to grow their responsibility

What You'll Do:

• Collaborates with Director of Nursing to develop Plan of Care for patients, implement IDT plans and attend IDT meetings
• Participates in all nursing and clinical services, including direct nursing care, patient care coordination, and QA of Plan of Care and nursing notes
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Coordinates patient care with different vendors such as nursing homes and pharmacies

Qualifications:

• Active and unrestricted LVN license in California
• CPR certification
• One (1) year of clinical experience
• Ability to drive to client sites in California
• Flexible schedule and weekend availability

Environmental Job Requirements and Working Conditions:

• This position is in office. The office is located at: 1680 S. Garfield Ave, Alhambra, CA 91801.
• Work is performed in an office environment and you will be required to travel to patient homes.
• Employee must be able to tolerate sitting or standing for long periods of time to perform different duties. Must be able to deal with busy, high pace work shifts. Pace of work may be rapid and intense especially when faced with deadlines. The noise level in the work environment is usually moderate.
