Duties & Responsibilities
Position Summary:
Under the guidance of the Program Manager, this position will serve as a supervisor over a team of medical and non-medical case management staff. Duties include technical support, monitoring accuracy in the application of program regulations, and identifying areas for process improvement to support the team. In addition, this position will hold their own caseload varying between non-medical case management and medical case management to adjust to the team's needs.
Core Responsibilities:
Responsibility 1: Performance Management
o Expectations are clearly communicated to staff and relate to the goals and objectives of the program with feedback solicited and constructive communication offered.
o Staff are provided with the necessary KSA's to do their job and continuing education is offered, expected, and supported.
o Performance issues are addressed and documented promptly and requirements are met.
o Complete and oversee annual performance evaluations and compliance training for staff by established deadlines with proper documentation.
o Meet with staff on a consistent basis to monitor progress and outcomes and adjust work plans as needed in order to meet goals.
Responsibility 2: Case Management
o Complete initial assessment and identification of various service needs, referral to RW support services, and determination of referral to MCM when conducting NMCM case management.
o Provide direct social work/case management services to People living with HIV/AIDS (PLWHA) and their families in accordance with their medical diagnosis and psychosocial, social and financial needs. Scope of services includes screening, assessment, crisis intervention, counseling, advocacy, information/referral, and care planning and coordination.
o Develop a comprehensive, individualized service plan when necessary.
o Respond to multiple referrals from community organizations, health department personnel, and community health providers to offer information regarding services
o Proficient in accurate documentation in clients Electronic Medical Record and Careware
o Provide assistance with the referral process by follow up of referral appointments

Position is exempt and is an in-person position.
Qualifications
Minimum Hiring Standards:
o Bachelor's degree in relevant discipline or equivalent work experience required
o Proven experience as supervisor or relevant role
o 5 years of Case Management experience
o 3 years of Supervision experience or equivalent people and program management experience
o Detailed knowledge of Ryan White Case Management standards
o Ability to work independently or as part of a team
o Must be able to perform daily activities with minimal supervision
o Must have excellent written and verbal communication skills; empathy, compassion, and confidence
o Must have ability to work and communicate effectively with individuals from culturally and socioeconomically diverse backgrounds
Preferred Hiring Standards:
o Master's degree in relevant discipline

o Excellent interpersonal and customer service skills
o Excellent oral presentation and writing skills
o Attention to detail
o Ability to multi-task
o Strong knowledge of program coordination and management
o Ability to meet specific deadlines and coordinate multiple tasks
