CPT was devoted exclusively to the delivery of home care services until 1996, when we expanded to include outpatient services and skilled nursing facility staffing and management services. Join Our Team as a Physical Therapist
Location: Alden Estates Barrington & Alden Poplar Creek
Status: Full-time (Monday - Friday, 40 hours per week)
About Community Physical Therapy (CPT):
At Community Physical Therapy (CPT), we are proud to be a leading provider of rehabilitation services in the greater Chicagoland and southern Wisconsin area. Our board-certified therapists offer Physical, Occupational, and Speech Therapies in various settings, including post-acute rehabilitation, skilled nursing facilities, home care, and outpatient services.

Job Responsibilities:
Adhere to CPT and facility policies and procedures to ensure exceptional therapy services.
Maintain high standards of care and rehabilitation services.
Assist with administrative duties necessary for the daily operations of the rehab department.
Build strong relationships with patients, their families, and third-party payers, ensuring quality care and communication.
Qualifications:
Graduate of an APTA-approved physical therapy program or its equivalent.
Licensed, certified, or registered to practice as a physical therapist in Illinois or Wisconsin.
Adhere to the APTA Code of Ethics and Standards of Practice.

Preferred Experience:
Experience in a Skilled Nursing Facility (SNF) is preferred.
Knowledge of Net Health is a plus, but we are happy to train the right candidate.
