About the Program:

The Supported Living Program (SL) provides community living services for adults with a wide range of disabilities and mental health challenges. The SL program promotes self-determination, independence, and competence for clients.

Essential Functions: (Tasks, Responsibilities, and Competencies. May not include all duties of this job)
  • Assessment: Makes basic observations, gathers data, and assists in the identification of needs and problems relevant to the resident; collects specific data as directed, and communicates outcomes of the data collection process in a timely fashion to the supervisor.
  • Problem Identification. Provides data to assist in the development of nursing diagnoses which are central to the plan of care.
  • Planning. Contributes to the development of approaches to meet the needs of the Clients and develops client care plans utilizing a standardized nursing care plan and assists in setting priorities for care.
  • Implementation. Carries out planned approaches to client care and performs common therapeutic techniques.
  • Evaluation. In collaboration with the RN, assists with making adjustments in the care plan. Reports outcomes of care to the RN.
  • Assists in health teaching of residents and provides routine health information and instruction recognizing individual differences.
  • Upon admission of resident, assists in assessing all health needs and problems; implement a plan of care; and re-evaluate the care plan as needed.
  • Assists in assessment and determination of resident abilities, strengths, support needs, interests and preferences in relation to all needs (personal care, physical health, medication, diet, physical and mental health) in relation to daily activities, routine and preferences. Overall health and wellbeing.
  • Supervise and provide medication administration. Administer injectable medications to assigned residents.
  • Collect bio-medical information from residents such as blood pressure, heart rate, weight, etc.
  • Regularly observe residents for changes in physical, mental and emotional functioning to determine if needs are being met.
  • Provide assistance for residents to raise awareness of need for health services, and consult with other resident support staff in observing, recognizing, and reporting health needs of residents. Encourage residents in health education, good nutrition, hydration and other activities related to good overall health and wellbeing.
  • Coordinate health care between the resident, their chosen health care providers, including the psychiatric provider*, and the Supported Living.
  • Timely notify all involved staff of changes in resident's medical needs, and instruct in the implementation of the changes (i.e., medications, activity limitation, special diet, etc.).
  • Notify RN and Director or appropriate members of the Clients health care team if significant changes in medical conditions occur which may potentially require additional support services. Communication shall take place in a time period consistent with the Clients need for care.
  • Provide supervision, management and administration of medication to Clients; provide instruction to Client and staff for the use of any and all prescribed medications and medical treatments including administering, storage and side effects. Pick-up or arrange for renewal of current medications, as well as arranging the disposal of outdated or discontinued medications.
  • Provide nutritional input for menus / meal planning to ensure resident needs are met.
  • Advocate with health care professionals to acquire the best health care options for Client. Facilitate the implementation of all follow-up care requirements.
  • Maintain charts and files for all residents on a regular basis including the documentation of all appointments, medications, correspondence, and conversations. Keep up-to-date health records that will be consistently and reliably available to authorized staff, Sunrise departments, community offices, County agencies, and the Department of Social and Health Services (DSHS). Maintain the health records of all dismissed or discharged residents for 6 years.
  • Document in health records all observed or reported illnesses, accidents, and changes in functional abilities as well as the dates of actions taken. Make necessary information available to authorized staff. Update charts and files for residents on a consistent and regular basis including the documentation of all client data, bio-medical information, medications, etc.
  • Ensure that all health records are maintained as privileged and confidential information pursuant to Sunrise policy, governmental regulations, and industry best practices. Allow confidential records to only be inspected by or transferred to authorized persons, such as legal guardians or persons possessing the resident's written consent.
  • Document case notes / incidents for each shift.
  • Keep medical rooms stocked with adequate supplies, as required.
  • Serve as Shift Lead for assigned shifts; provide support and leadership to Direct Care Supervisor, Direct Support Professional on shift in coordination with or the absence of the Program Manager/Supported Living Director to ensure correct follow-up actions and notifications regarding incidents to outside agencies and individuals as required by Sunrise Supported Living Incident Reporting policy; respond to any scheduling/staff "call in" needs; ensure housekeeping duties for the shift are completed.
  • Participate in any investigation of any accident or incident which jeopardized the health, safety or life of a Client and institute measures to prevent similar occurrences in the future as related to health matters.
  • Promptly notify the client next of kin, guardian, or other individual or agency responsible for the resident, upon the occurrence of emergent events such as serious changes in the resident's condition, or transfer of the resident to hospital.
  • Screens visitors to the facility on after-hours shifts.
  • Assist direct care staff with daily activities of the facility, e.g. Clients morning and evening routines, etc. by cueing, encouraging and reminding residents of tasks related to those activities; housekeeping, food / meal preparation, and problem solving and conflict resolution.
  • Participate in daily staff organizational meetings and treatment planning review meetings.
  • Maintain all on-going licensing and certification requirements for this position and supply Sunrise proof of the same.
  • Obtain instruction, supervision and consultation as necessary before implementing new or unfamiliar techniques or procedures which are in his / her scope of practice.
  • Perform other duties as assigned.
Essential Requirements: (Any Equivalent Combination of Knowledge, Skills, Abilities, Education, and Experience)
  • Education: AA degree in nursing or other required nursing education for an LPN.
  • Experience: Two years of experience in providing health care and support to, mentally ill, physically disabled or developmentally disabled individuals. Any other acceptable combination of education and experience, which insures the ability to perform the duties of this position may be substituted for the above requirements.
  • Licensure/Certification: Licensed Practical Nurse (LPN) per WAC 246-840.
Other Considerations:
  • Current valid Washington driver's license, with excellent driving record and $50,000 liability insurance.
  • DSHS background clearance required.
  • Basic computer skills including Microsoft Office Word and Excel.
  • Perform simple math functions with or without a calculator.
  • Strong verbal and written communication skills, and demonstrated leadership ability.
  • Demonstrated ability to exercise sound judgment in challenging dynamic situations including behavioral and staffing issues.
  • Sit or stand for extended periods of time, perform light lifting (approximately 10-20 pounds), and physically assist unsteady residents.
  • Must be able to perform physical aspects of position that may include carrying groceries, laundry, and other personal belongings; reaching at waist, above shoulders or below knees, walking up and down stairs, etc.
Compensation:
  • $30-$35/hour
  • All wages are determined by applicable industry experience, education, and
    certifications/licensure.
