Pharmacist III

Employment Type: Full time Shift: Day Shift Description: Position Title: Pharmacist III- Home Infusion Hours of Operation are M - F 8:30 am - 5 pm This position is part time, 20 hours per week May require occasional weekend or extended hours Home Infusion Pharmacy located at Cathedral Square building benefit eligible Position Summary: Routinely performs all of the duties of a staff pharmacist but also utilizes a large amount of their assigned work time to a specialty area within the pharmacy that provides direct support or training for other staff. This position often requires odd work schedules, off site meetings, on-call time and weekend shifts in order to maintain software systems while not disrupting departmental operations. What the Pharmacist III- Home Infusion will need: Five year bachelor of science degree in pharmacy from an accredited college of pharmacy. Current registered pharmacist license and controlled substance license issued by the Michigan State Board of Pharmacy. Five or more years of work in a hospital or specialty pharmacy preferred Ideal candidate has home infusion experience; IV antibiotics and nutrition experience What the Pharmacist III- Home Infusion will do: Reviews, prepares and dispenses medication orders per physician order or prescription according to established policies, procedures, and protocols. Ensures safe, appropriate, cost effective medication therapies for patients according to established policies, procedures and protocols. Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to physicians, as needed. Provides drug information and counseling to patients, families, and other healthcare professionals.
