What does a Admissions Coordinator do?

As the Admissions Coordinator at Four Winds Manor, Inc., you will play a pivotal role in arranging all new admissions to our facility, ensuring a smooth transition for residents. Your responsibilities will extend to coordinating care for short-term rehab residents, which includes facilitating crucial care conferences with families and residents. By attending weekly Medicare meetings and conducting appropriate MDS assessments, you'll help to develop personalized care plans that reflect our commitment to empathy and compassion.

Additionally, you will set up discharge services to ensure residents leave with the support they need, all while fostering strong relationships through follow-up and effective communication. Your innovative approach will empower families to trust in our high-performance team, making a significant impact on each resident's journey toward recovery.

Would you be a great Admissions Coordinator?

To excel as an Admissions Coordinator at Four Winds Manor, Inc., strong problem-solving abilities and a customer-centric mindset are essential. A Bachelor's degree is preferred, along with at least one year of experience in a long-term care or healthcare setting, though this is not a strict requirement. Success in this role hinges on excellent networking skills and relationship building, as you will be interacting with families and residents daily. Proficiency in relevant software tools is crucial for conducting MDS assessments and developing care plans effectively.

Your ability to show empathy and compassion while navigating care conferences will help establish trust and rapport with families. Additionally, strong follow-up skills are necessary to ensure all discharge services are set up seamlessly, allowing you to contribute positively to the residents' overall experience in our dynamic and forward-thinking environment.

Knowledge and skills required for the position are:

  • Bachelor's degree (preferred)
  • At least 1-year experience in long-term care or healthcare setting preferred though not necessary
