Gentiva is excited to invite applications for the role of Nurse Practitioner specializing in Hospice Care on a per diem basis. Our organization plays a vital role in providing comfort and support to patients and their families in their time of need.

As a Nurse Practitioner at Gentiva Hospice, you will collaborate closely with our Executive Director or Administrator, focusing on critical Face-to-Face (F2F) encounters. Your expertise will be pivotal in gathering essential clinical information that supports hospice diagnosis and prognosis, which will then be communicated effectively to the hospice physician for eligibility certification.

Key Responsibilities:

  • Conduct comprehensive physical assessments for F2F encounters, adhering to regulatory time frames established by CMS.
  • Deliver clinical findings derived from medical treatment plans, diagnostic tests, lab results, and medication lists to the certifying hospice physician.
  • Provide insightful recommendations to the Interdisciplinary Group (IDG) based on the outcomes of your assessments.
  • Communicate promptly with the hospice agency if you are unable to complete a scheduled F2F visit.

Qualifications:

  • Master's or higher degree in Nursing.
  • Graduated from an accredited nurse practitioner education program by a recognized national body.
  • Possess a current state license as a registered nurse.
  • Hold a current state license or certificate as an advanced practice nurse.
  • Have national certification as a Nurse Practitioner (ANCC or AANP).
  • Present a collaborative agreement.
