QUALIFICATIONS
  1. Licensed Practical Nurse or Registered Nurse with degree from an accredited nursing school and holds a current RN or LPN license in the State of Michigan.
  2. A minimum of three years of nursing practice, with at least one year in hospice or home care is preferred.
  3. Possesses and maintains a current CPR certification.
  4. Maintains a valid Michigan driver's license and has the availability of a reliable, licensed, and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.
SUMMARY OF JOB RESPONSIBILITIES

The Triage Support Nurse prioritizes and responds to calls related to patient care received, in a timely manner. Incorporates nursing skills and appropriate questions to accurately evaluate problems/needs and provide education to the patient/caregiver in accordance with the plan of care. Provides emotional support appropriately during telephone conversations to patients/caregivers. Determines appropriate discipline to respond to call and/or visit patient/family. Conducts follow up calls and/or visits as problems/needs arise or as requested by supervisor.

ESSENTIAL FUNCTIONS AND DUTIES (other duties may be assigned)
  1. Maintains the confidentiality of patient and employee information at all times, both at work and off duty, and after the termination of employment, according to the agencies Notice of Privacy Practices and HIPAA regulations.
  2. Is knowledgeable and functional in computer systems including Microsoft Applications, Teams, Electronic Medical Record (EMR) and e-mail system as required for the execution of the job.
  3. Is knowledgeable of hospice concepts and philosophy.
  4. Proficient in telephone techniques including customer service, phone etiquette, and handling of calls.
  5. Demonstrated proficiency working within a fast-paced team environment and able to apply critical thinking and problem-solving skills independently.
  6. Obtains and assesses clinical information regarding patients' condition and responds accordingly and timely to accepted standards of nursing practice.
  7. Communicates with other members of clinical team and resources with accurate information reflecting knowledge of hospice services, hospice regulations and Harbor Hospice policies and procedures. Follows up with concerns and brings unresolved issues to Clinical Manager.
  8. Assists Clinical Manager with daily schedules and calendars related to patient care.
  9. Works cooperatively on projects and tasks with the Clinical Manager in maintaining compliance with Medicare/Medicaid, State and Federal Regulations, NHPCO and Joint Commission standards and other relevant projects as requested.
  10. Provide in field patient care/visits as needed to assure effective coverage.
  11. Participate in on call schedule as needed including weekend rotation.
  12. Reviews and follows up from daily on call report.
  13. Accepts orders from providers, confirms in writing according to hospice policy and routes information to appropriate caregiver/team including notifying staff in field as needed. Documents medication orders through e-prescribing platform and orders DME from approved vendors.
  14. Documents all interactions with patients, families, contacts, and vendors in the EMR.
  15. Assists Clinical Educator with monitoring completion of employee education and competencies.
  16. Performs FIT testing and assists with CPR certification training for new and current staff.
  17. Assists with new hire employee orientation including preparing schedules and binders.
  18. Performs other duties as assigned by Clinical Manager.
