Pay: $27 to $32

- Facilitates communication between nursing center departments and patient/family members to provide an effective assessment/care plan process.
- Establishes open lines of communication with consultants.
- Has effective interviewing skills for the purpose of collecting information from patients, families and staff members.
- Assists Director with patient or family interviews for assessment purposes.
- Assists Director with evaluation of each patient's condition and medical data to determine special needs, or the need to amend the admission assessment.
- Has a comprehensive understanding of nursing resource material and understands how to incorporate such information into care plans [i.e., nursing journal articles that relate to specific patient care topics (bowel and bladder retraining, restraint assessment and use, fall assessments, skin assessments, etc.)].
- Establishes open lines of communication with interdisciplinary team members in order to provide a comprehensive and holistic patient care plan.
- Maintains schedule as assessment/care plan meetings. Coordinates schedule with nursing center staff and others (family, therapists, etc.) to provide for appropriate participation.
- Helps to provide that care is provided in accordance with patient's wishes.
- Assists Director with coordinating the development of a care plan (preliminary and comprehensive) for each patient that identifies problems/needs, appropriate interventions and measurable goals.
- Helps to provide that patient assessment and care plans are completed in a timeframe that is in accordance with current regulations.
- Assists Director with facilitation of education to inform associates of the patient's care plan and review care plan prior to administering care.
- Coordinates the review of each assessment/care plan for accuracy including timeliness, appropriate signatures, dates, etc.
- Helps to provide that each assessment is signed and dated to certify its completion.
- Monitors the MDS process for accuracy according to the MDS guidelines and other accepted standards.
- Assists Director in education of the interdisciplinary team and other staff members as needed in the MDS process.
- Assists Director with physical assessment of patient to provide accurate MDS process as requested.
- May be requested to assist with nursing service educational programs in reference to patient assessment and care.
- May be requested to assist with meeting all shifts for the purposes or planning, coordinating and implementing patient care.
- Assists in compliance with Federal, State and local regulations in relation to scheduling, performing and implementing patient care plans.
- May be required to assist with management reports and participate in special assignments.
- Complies with specific time limitations in terms of assistance given to Director in scheduling and planning patient care.
- Promotes the image and reputation of the System by exhibiting servant leadership and providing direct and open lines of communication.
- Contributes to the work of committees, workgroups, project management, and other collaborative efforts of the System.
- Performs other duties as necessary to ensure the success of the System.
- Proficient in using a computer, especially Microsoft Office, Excel and Power Point.
- Excellent organizational and prioritizing skills required

- Communicates well with other associates, patients, family members, and visitors providing warm and friendly greeting and an approachable attitude. Responds to expressed concerns while displaying a helpful, caring demeanor, answers questions when appropriate in a professional manner.
MINIMUM QUALIFICATIONS
Nursing Degree from an accredited college or university or is a graduate of an approved LPN program.
An active LPN License to practice in the State of Georgia
SUPERVISORY RESPONSIBILITIES
None.
LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or associates of the organization.
MATHEMATICAL SKILLS
Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals.
REASONING ABILITY
Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram format. The ability to deal with problems involving several concrete variables in standardized situations.
