ESSENTIAL DUTIES:

  • Enable patients to stay independent in their own homes
  • Perform routine supervisory visits and evaluate the plan of care
  • Provide educational in-service trainings
  • Communicate with Case Managers and other care team members

POSITION REQUIREMENTS & COMPETENCIES:

  • Licensed as a vocational/practical nurse in the state of practice
  • Two (2) years clinical practical nursing experience
  • Ability to make home visits and provide nursing care
  • Complete necessary documentation appropriately and timely
  • Reliable transportation, current driver's license and appropriate automobile insurance; or ability to take public transportation
