What You Will Do: Provides the full range of behavioral health treatment, including diagnostic assessment; individual, family and group therapy; and crisis intervention. Home-based therapy services will be provided to target populations. The scope and volume of services provided will vary as related to specific job role. Performs administrative tasks as assigned by supervisor including development of referral protocols for home-based therapy services and procedures for provision of services. Markets home-based services program to key stakeholders and referral sources. Collaborates with Marketing department regarding development of brochures and other media to promote program. Responsible for complying with all requirements of the continuous quality improvement program. Independently diagnoses and treats behavioral health disorders within scope of practice. Renders all services within department guidelines and in compliance with rules/standards set forth by Medicaid/Medicare, HFAP, OhioMHAS, and CARF. Demonstrates a commitment to the provision of quality care through engagement in professional growth and agency-sponsored learning opportunities; utilizes best practices as appropriate to the conditions being treated; and provides all services in a manner that instills hope and meets patients where they are at in terms of their readiness for change. Uses best practices for patient engagement and retention throughout the episode of care. Screens referrals for home-based services to ensure clinically appropriate. Coordinates with supervisor and front desk staff regarding opening persons to home-based behavioral health services. Provides case management services as needed to home-based clients in addition to therapy services. Facilitates direct referrals to inpatient psychiatric care when clinically indicated. Facilitates referrals to social services, healthcare, and other services based on needs identified through initial and ongoing assessment. Performs tasks that provide for patients’ safety including ongoing screening and documentation of risk factors; develops safety plans and crisis management plans when indicated; and coordinates with afterhours services as needed to promote patient care. Understands the role of accurate and timely clinical documentation in relation to quality care and patient safety. Reviews crisis services documentation as needed. Is able to identify and provide feedback to clinicians to promote quality of patient care and safety. Promotes patient recovery through active and collaborative patient involvement in initial and ongoing treatment planning. Fosters recovery and independence by identifying and incorporating patients’ strengths and abilities into the treatment process. Involves natural support persons, self-help resources, and other community integration activities as critical components to long-term recovery. Decreases length and frequency of sessions as patients prepare for discharge. Is able to effectively instruct other clinicians, as needed, regarding clinically appropriate methods for decreasing and/or increasing frequency of sessions in response to medical necessity. Understands the role of Patient Care Standards (PCS) as a measure of our commitment to serving those in need and ensuring that access to care is provided. Ensures PCS standards are met and actively utilizes and responds to strategies provided through supervision when there are challenges in meeting these standards. Is able to effectively communicate the rationale behind PCS expectations to other employees. Is able to help other employees identify methods for achieving PCS expectations. Performs tasks necessary to support revenue including timely and accurate documentation within departmental guidelines; maintaining current treatment plans; and complying with ongoing prior-authorizations for services when applicable. Reviews clinician documentation, as assigned, to ensure documentation complies with policies and procedures and promotes quality care. Is able to teach others skills needed regarding documentation practices. Effectively and efficiently utilizes EMR and video-conferencing software. Utilizes work email on a daily basis keeping current with FRMC and departmental communications. For new hires, these skills are in place by the end of the probationary period. Ensures all electronic forms of communication, that include PHI, are compliant with HIPAA and 42 CFR laws regarding confidentiality. Is able to teach others these skills as assigned. What You Will Need: Therapists rendering mental health services and substance use services are required to hold an active, independent counselor or social worker license through the Ohio CSWMFT Board. Heart Saver, Aegis Crisis Prevention, and First Aid certification within 6 months of hire. Ability to effectively understand and communicate in both written and oral form. Sensitive to the cultural needs of the individual and/or family served. Punctual attendance at assigned work location is required. Ability to work in a safe and efficient manner and maintain an accident free work place, including ability to demonstrate a working knowledge of emergency codes. Successful completion of a 180-day probationary period.
