Essential Job Duties:

- Conduct initial and ongoing age-specific assessments in designated time frames.
- Contribute to Interdisciplinary Plan of care using assessment data.
- Evaluate effectiveness of plan/of care and modifies, as appropriate.
- Involve patient/family/significant other in developing plan of care.
- Collaborate with other clinical disciplines in developing plan of care.
- Provide care based on scientific principles/nursing theory and evidence based practice.
- Recognize change in the patient's condition and intervene appropriately.
- Prioritize nursing actions, reflecting patient needs, acuity, and desired outcomes.
- Implement physician orders accurately within appropriate time frames.
- Demonstrate competency in core & unit-specific clinical skills.
- Identify continuing care needs of the patient for the discharge plan.
- In the absence of a front desk coordinator, scheduling patients for HBO therapy or wound care visit, utilizing hospital software/ systems as well as RXH's EMR system may be required.
- In the absence of a front desk coordinator, registration to be accomplished and in accordance with hospital policy and software / systems requirements as well as RXH's EMR system.
- All patient treatment notes to be entered into RXH's EMR system.
- Provide patient/family education to include:
- Assessment of learning needs, abilities and readiness to learn,
- Documentation of teaching activities,
- Provision of instruction as appropriat e.g. painmanagement, medication, diet, equipment, activities after discharge
- Provision of appropriate educational materials, as available prior to discharge.
- Document all care provided accurately & completely according to Hospital and Department Policy.
- HIPAA Compliance - complies with all of the Privacy and Confidentiality Policy and Procedures outlined in the "Privacy, Security and Confidentiality Agreement.
- Adheres to all Patient Safety Initiatives and Infection Control Procedures as outlined in the hospital's safety manual. Appropriate use of Hospital resources, supplies and equipment.
- Process and utilize all computerized patient care functions, e.g. ADT, diagnostic test results, etc.
- Apply for nursing council or committee and be vested on one council at least every 5 years. 80% attendance in person or by phone is required. Advancements in Clinical Ladder will be reflected in yearly application process and is expected for job performance standards
- Completes other duties and special projects as assigned.
Qualifications
- Must have and maintain a valid CPR certification from the American Heart Association.
- Must have and maintain a valid Nurse Practitioner (NP) or Registered Nurse (RN) license in the state in which the Nurse will work; Bachelors in Nursing within 3 years of employment (with the exception of staff previously granted grandfather status).
- Must have prior experience working with wound care patients.
- Prior experience with billing and coding preferred.
