Hospice Care Transition Coordinator- Jeffersonville, IN

As a Hospice Care Transition Coordinator (CTC), you will be called to care when you’re needed most. As part of Interim HealthCare, you’ll support a full range of patient services to bring comfort and dignity to our clients.

As a Hospice Care Transition Coordinator, you will:
- Serve as a subject matter expert for patients and families moving from a healthcare facility to in-home care/hospice
- Act as a patient/family liaison to providers and health plans when navigating the home care transition
- Evaluate transitions from home care to hospice, palliative to hospice, and hospice to palliative
- Perform Admissions if needed
- Field customer service inquiries of a clinical nature
- Current unrestricted license to practice as an Registered Nurse (RN) or Social Worker
- One (1) year of professional experience practicing as Registered Nurse (RN) or Social Worker
- Reliable transportation to/from care sites and/or work locations
- Previous experience as a home care intake specialist or case manager preferred
