Job Overview:
The Pediatric Registered Dietician / Diabetes Educator (RD/CDCES) is a registered dietician who assumes clinical responsibility for education of pediatric patients with diabetes and their families about diabetes self-management skills with a focus on nutrition. The RD/CDCES also will see general nutrition patients for endocrine-related nutrition disorders such as obesity, failure to thrive, hyperlipidemia, and pre-diabetes.
Reporting to the Medical Group Regional Manager, and working under the direction of a physician, the RD/CDCES will assess the patient s past history and current condition, establish and monitor the patient/family goals and outcomes; coordinate, monitor and evaluate patient care in the outpatient setting.
Qualifications:
Required:
- Graduate from an accredited dietician program.
- Current CDCES preferred
- Current New Jersey Dietician License
Preferred:
- Current CDCES preferred but will train the right candidate
Certifications and Licenses Required:
- Current NJ Dietician License
Scheduling Requirements:
- Shift working, 9:00 4:30 Monday, Tuesday, Thursday, and Friday
- Part-Time
Essential Functions:
- Assess and implement patient/family education and home care needs; use an understanding of the principles of growth and development to assess patients; age specific needs and age specific treatment.
- Specifically responsible for review and management of patient blood sugar levels, including application of the Continuous Glucose Monitoring System.
- Plan and conduct patient education classes as necessary.
- Anticipate and evaluate patient problems, establish outcomes and produce a plan of care in collaboration with the patient/ family.
- Assist in the development of and participate in appropriate educational programs.
- Recognize and act upon need for review and study of materials which will enhance knowledge and performance.
- Consults with physician regarding medical orders on participants, i.e., insulin dosing and diet.
- Consults with the family and patient from home to assist with education and problem solving.
- Encourage and support patient/family self-direction and independence while providing education.
- Maintains patient right to privacy and confidentiality
- Make recommendations of patient care based on: the medically prescribed treatment plan, RD/CDCES assessment, patient/family input, collaboration with other health team members and coordinate all the services with Physician requests.
- Monitors medical needs of patients and documents same in medical records.
- Observe, record and report signs and symptoms of the disease process, changes in patient s physical, emotional and social condition and responses to the therapeutic regimen.
- Provides education and training for participants and/or families as required.
- Reviews blood sugar readings, documenting pertinent data on the chart, and brings abnormal results to the attention of the physician.
- Takes an active role in maintaining current knowledge based through reading, attending conferences and continuing education course work, sharing the information gained with coworkers through follow-up presentations.
- Teach, demonstrate and supervise patient care techniques regarding insulin administration
