POSITION SUMMARY: As a member of the interdisciplinary team, the LVN works under the general direction of the Patient Care Manager and the Registered Nurse/Case Manager to gather patient information and to provide treatments and care within their scope of practice in order to maximize the comfort and health of patients and families.

RESPONSIBILITIES:

  • Performs scheduled home visits to gather patient/family information for the RN/case manager and provide teaching and perform patient care tasks and treatments under the direction of the RN/case manager or Patient Care Manager.
    • Standard: Home Visits made according to plan of care and under the direction of the RN case manager. Documentation completed at time of visit and submitted with the time sheet on the first work day following the visit.
  • Maintains regular communication to RN/case manager and/or Patient Care Manager and keep informed of significant patient changes and any unusual problematic patient/family issues.
    • Standard: Patient's changes and needs to be called to the RN/case manager or the Patient Care Manager at the time of the LVN visit.
  • Attends regularly scheduled care conferences (IDT) to review cases, problem-solve and cover unique issues, share professional support and exchange feedback aimed toward enhancing professional growth.
    • Standard: Attendance at weekly conference encouraged, unless assigned essential patient care by the Patient Care Manager. Attend joint care planning meetings as requested when assigned to the SNF/RCFE team.
  • Participates in in-services, workshops, and clinical meetings to promote professional growth and understanding of hospice care
    • Standard: Attendance at 80% of all mandatory scheduled in-services and meetings, designated as LVN appropriate, provided by agency. Completes all required in-services.
  • Submits visit notes and timesheets in a timely manner.
    • Standard: Submit visit notes and timesheets within 24 hours of visit.
  • Cooperates with regulatory requirements for supervisory visits by the case manager.
    • Standard: Schedule visit appointments and reliably adhere to schedule so that supervisory visits can be completed in a timely manner.
  • Follows all Community Hospice policies and procedures including, but not limited to, safety, infection control, privacy and confidentiality, and HIPAA.
    • Standard: 100% Compliance.
  • Must maintain LVN license in accordance with federal, state and local laws, and must act only within the scope of his or her state license. All personnel qualifications must be kept current at all times.
    • Standard: 100% Compliance.
  • Maintains a professional appearance of self and working area; exhibits appropriate behavior with patients and families, i.e., courteous, thoughtful and professional at all times.
  • Participates in Quality Improvement efforts as assigned by the Patient Care Manager.
  • All other duties as assigned by the Patient Care Manager.
Qualifications
  • Graduation of an accredited school of nursing LVN program.
  • One year current LVN experience. Wound and skin care experience highly desirable.
  • Current California LVN license.
  • Current California driver's license and automobile insurance.
  • CPR certification.
