PRIMARY FUNCTIONS: The Clinic Navigator assists the nurse coordinator and other members of the medical team with a variety of duties essential to daily clinic operations and patient care.

ESSENTIAL DUTIES/ RESPONSIBILITIES:
• Acts as member of the clinical team to support co-workers and ensure patient needs are met.
• Answers phone calls from patients, staff, and outside agencies.
• Takes phone messages and communicates messages to the nurse coordinator or other team members as appropriate.
• Assists patients with making appointments.
• Follows up with patients as necessary to maintain continuity of care.
• Obtains records from other healthcare providers as needed.
• Utilizes time productively by addressing telephone encounters and patient documents in a timely manner.
• Conducts vaccine inventory and places orders as needed.
• Routinely stocks patient exam rooms with needed supplies.
• Maintains medical supply inventory and places orders as needed.
• Performs and documents quality control on waived testing equipment as required.
• Consistently interacts with patients and staff in a manner that reflects favorably on the organization and promotes teamwork.
• Actively participates in team huddles and assists in process improvement.
• Performs other duties as necessary and may rotate to other sites as determined by nurse coordinator.

QUALIFICATION/EDUCATION and/or EXPERIENCE REQUIREMENTS:
• High school diploma or general education degree required.
• Good oral and written communication skills in English. Bilingual in Spanish preferred.
• Attention to detail and excellent technical skills.
• Good interpersonal skills.
