A clinical nutrition professional with expertise in providing direct patient care through the application of the Nutrition Care Process (NCP), including nutrition assessment, diagnosis, intervention, and monitoring. Responsible for nutrition counseling and education for inpatient and outpatient populations.

  • Perform Nutrition Care Process and Nutrition Focused Physical Exam as appropriate for high quality patient care
  • Collaborate with interdisciplinary team to advocate for patient care and support a culture of nutrition
  • Maintain accurate record keeping of clinical activities and contribute to improving department and organizational metrics
Required Qualifications:
  • Bachelor's Degree in Nutrition, Dietetics, or related field
  • CRT-Registered Dietitian (RD) - CDR Commission on Dietetic Registration 120 Days required
  • LIC-Driver's License - STATE_MI State of Michigan Home Care Business Lines required
  • CRT-Basic Life Support (BLS) - AHA American Heart Association SH Lakeland Homecare 30 Days required Or CRT-Basic Life Support (BLS) - ARC American Red Cross SH Lakeland Homecare 30 Days required
