Public Health Nurse-PAT

JOB REQUIREMENTS: Are you interested in working with moms and babies? Kenosha County Public Health is looking for self-directed, nurturing, and adaptable individuals to join our team of Public Health Nurses. The Public Health Nurse (PHN)-Home Visitation provides nurse home visits, implementing two home visitation programs: Parents as Teachers (PAT) and Hello Baby Kenosha County. The PHN-PAT works throughout the perinatal period to improve birth outcomes; promote child health, safety, and development; improve maternal health; and improve service coordination with and referral to other community resources and supports. The PHN-Hello Baby works to enhance family and child well-being through postpartum home visits to every family with a newborn, offering supportive guidance, responding to family questions about newborn care, and helping bridge the gap between parent needs and community resources. The position ensures compliance with federal, state, and local regulations and requirements. This is a grant-funded position and is contingent upon continued funding. Bright Family Beginnings (Nurse Home Visitation Program) Provide screenings virtually and via home visits to women and their families eligible for evidence-based programs via the Family Foundations state grant Carry a maximum caseload of up to 9 families as defined by model program(s) and follow newly delivered families for short interval appointments. Assist families in establishing goals and outcomes with educational supports Educate and provide guidance to parents on implementation of proper developmental activities for their children Identify populations negatively affected by events and crisis and the conditions, systems, and structures that support these outcomes Develop and maintain cooperative working relationships with first responder agencies, local community part Nursing Services Perform initial intake assessments and conduct appropriate health screenings for clients Aid in the assessment, coordination, planning, and evaluation of care plans and treatment goals Research and refer clients for additional community services and resources, as needed Assist clients in enrolling in state assistance programs Perform pregnancy testing and provide contraception counseling and management, referrals, and education on family planning and contraception Adhere to the nursing process and the fidelity of model program(s) Ensure adherence and compliance to the policies and procedures of the federal Health Insurance Portability and Accountability Act (HIPAA) regarding protected health information Health Promotion and Community Health Improvement Initiatives Provide education and community awareness on behalf of Kenosha County Public Health while networking with other public and private community groups, coalitions, and organizations (i.e., health-related agencies, courts, schools, etc.) Participate in the planning, development, and implementation of public health policies, procedures, programs, and services Collaborate with other public health programs and community organizations to strategically develop collective impact strategies Participate in off-site mobile health missions in a variety of capacities Assist with identifying, developing, and advancing public health interventions to improve social determinants of health and health equity in our community Provide gender-inclusive sexual and reproductive health care services to the community through education, disease prevention interventions, and anticipatory guidance counseling Participate in the prevention of STIs, HIV, and other communicable diseases through one-on-one counseling, group education, and community outreach Collaborate with other teams in Kenosha County Public Health to provide education on the Community Health Improvement Plan (CHIP) and Strategic Plan Kenosha County is committed to Equity, Diversity and Inclusion and is an Equal Opportunity Employer. ** *** OTHER EXPERIENCE AND QUALIFICATIONS: Knowledge of: Infant and child development Principles and practices of professional nursing Health promotion and health prevention Population health practices and community health improvement Health equity, racial equity, and the social determinants of health Mental health conditions and treatment options Skill in: Critical thinking, systems thinking, analysis, problem-solving Time management and prioritization Oral and written communication Use of Microsoft Office, including Word, Excel, Outlook, and PowerPoint; basic internet Ability to: Work independently, making nursing decisions that are within the scope of practice Effectively interact with persons from diverse cultural, socioeconomic, educational, racial, ethnic, and professional backgrounds, and persons of all ages, gender identities, sexual orientations, and lifestyles Effectively work in a team environment that promotes customer satisfaction as well as independently to meet deadlines Remain professional and courteous at all times, especially in high pressure situations Effectively plan, organize, and manage competing priorities and challenges under pressure Engage, establish, and maintain collaborative partnerships with multi-cultural/multi-disciplinary staff, management, community partners, and the public Maintain confidentiality and serve the public with honesty and integrity Work respectfully in a diverse and inclusive environment ***** APPLICATION INSTRUCTIONS: Other: To see the complete job description and to apply for this position, please visit our website, www.kenoshacounty.org/jobs, and click the \"View Open Jobs\" button.
