Employment Type: Part time Shift: Day Shift Description: This position is for a RN Case Manager at Trinity Health Hospital in Grand Rapids, MI. Trinity Health Grand Rapids is a Magnet Designated Hospital This role will provide case management to our patients and families in the acute care setting for the older adult population unit. Paid holidays and generous Paid Time Off (PTO) Up to 18 days of PTO your first year Daily Pay Options Accredited transition-to-practice Residency Program from the American Nurses Credentialing Center (ANCC) Job Summary: Oversees and coordinates the discharge planning and utilization management activities for assigned caseload. Facilitates appropriate utilization of resources and efficient patient progression through the continuum of health care resulting in the highest quality and most cost-effective care. What you will need: Minimum: BSN; or ADN, with BSN to be obtained within 4 years of hire; ADN only, if nationally certified in Case Management. Current RN license in the State of Michigan. Active BLS Certification Preferred: Certification in Case Management or other related professional certification. Two to five years of medical/surgery or chronic disease management experience in an acute care setting Preferred: Previous case management experience, experience as a participant in continuous quality improvement, completion of self-management support training.
