  • Refer patients to additional mental health services, including psychiatric care and psychotropic medication management, when appropriate.

  • Participate in integrated systems of care, including staff meetings, case reviews, and supervision, to ensure a collaborative approach to patient care.
  • Maintain detailed, systematic documentation of patient encounters and treatment plans in the electronic medical record (EMR), ensuring timely and accurate records for clinical and billing purposes.

Additional Duties

  • Participate in street rounds with the outreach team and attend case conferences and team meetings as required.
  • Assist with coordinating care and referrals with external agencies, including shelters, harm reduction programs, and mental health services.
  • Maintain professional development and participate in continuous learning opportunities related to street medicine, behavioral health, and trauma-informed care.
  • Report suspected cases of child, elder, dependent adult, or domestic abuse in compliance with applicable legal mandates (Penal Code, Section 11166, Section 15630, and Section 1160).
