ESSENTIAL DUTIES:

  • Perform selected nursing procedures within the scope of LPN/LVN practice
  • Provide patient and family teaching according to plan outlined by the RN
  • Monitor vital signs, intake/output, and patient's condition
  • Administer medications as instructed and document patient's file
  • Coordinate patient care with the health care team
  • Maintain confidentiality and comply with regulations

POSITION REQUIREMENTS & COMPETENCIES:

  • Graduate from an accredited school of practical nursing
  • Licensed as an LPN in the state of practice
  • At least two years of clinical practical nursing experience
  • Ability to make home visits and provide nursing care in the home environment
  • Excellent interpersonal, verbal, and written communication skills
  • Reliable transportation and current driver's license
