Shift Information: Monday-Friday 8a-5p
On Call: 1x weekly as second and 1 weekend every month

Your Responsibilities:
- Manage a primary caseload for patients and families, encompassing assessment, planning, implementation, and evaluation of nursing processes.
- Gather and evaluate patient information, considering physical, psychological, social, and spiritual factors impacting their needs.
- Coordinate with physicians, interdisciplinary teams, and other agencies for optimal patient care.
- Conduct regular reassessments of patient needs and update care plans accordingly.
- Perform clinical pronouncement of death and provide compassionate support to families.
- Maintain accurate medical records, charts, and timely care plans.
- Supervise and document the care provided by Home Health Aides in compliance with guidelines.
Qualifications:
- Graduate from an accredited nursing program.
- Current RN license in the state of practice.
- CPR and First Aid certification required.
- Knowledgeable in quality assessment and assurance procedures.
- Valid driver's license and proof of insurance are mandatory.
