The Patient Care coordinator is responsible for set up andcare to Consumers of All American home Care who require care and support intheir homes. The Patient Care coordinator also is responsible for supervisionof Home Care aides who provide and support patients/Consumers with personalcare.

Qualifications and EducationRequirements

• Minimum of Associates Degree required,Bachelorâ€'s Degree preferred or combination of experience.

• 2+ years experience in a Case Management supportrole to leadership staff. Human Resources office experience preferred.

• Demonstrated capability maintaining strictconfidentiality with employee information.

• Knowledge of principles and practices of basicoffice management and organization.

• Strong typing and computer application skills.

• Computer proficiency and working knowledge ofMicrosoft programs required.

• Strong interpersonal and business partneringskills.

• Good judgement and decision-making skills.

• Must be well organized, accurate and attentiveto detail.

• Excellent communication, public relations andfollow up skills.

• Must be able to work independently and havestrong written and verbal communication skills.

• Belief in the Home Care concept.

Duties and Responsibilities

A. Musthave reliable transportation in order to travel to participants homes,meetings, events and any other work-related need.

B. Conductingvisits with participant and reviewing the Service Plan according to participantdisabilities and needs with assisting with activities of daily living.

C. Assuringthat the service plan is appropriately developed with participant andimplemented.

Coordinate and monitor the provision of services with a goal ofincreasing independence, productivity and integration into the community whilemaintaining the health and welfare of individuals.

D. Supporting the participant to become aneffective advocate and problem solver.

E. Welcome Calls to consumer.

F. 30 day Follow up calls to consumers.

G. Tracks the Home Health Aide notes & preparethem for payroll.

H. Makes sure the home Health Aides are followingthe Plan of Care.

I. Schedules and Reschedules Home Health Aides fornew and existing cases.

J. Initiates and oversees the assessment of serviceplan.

K. Conducts in-home visits no less than once per 60days.

L. Complete or monitor incident reports as requiredby the regulating entities.

M. Maintain accurate records for each participantin compliance with all regulations put forth by regulating entities.

N. Attend appointments and meetings as necessary.

O. Assume the responsibility of the on-call phonefor 2 weeks on a rotating basis and assist, advocate and counsel participantsas needed.

P. Perform other duties upon request.
