The goal of GTP is to reduce the length of inpatient stay, stabilize housing, and prevent individuals from being discharged from facilities or losing their residence and from being admitted to the hospital and/or presenting to the emergency departments due to their mental illness.

General Responsibilities: Provide services to Geriatric Transitions. Provide intensive 1-1 interventions and support to problem-solve and facilitate successful client transitions from hospital settings to community placements. Provide consultation and training to residential provider staff to create and or stabilize successful community placements. Coordinate client treatment and effective support with client's prescriber and others involved in client's care and needs including other MH Providers, DSHS, hospital staff, residential providers, family members etc.

Essential Functions: (Tasks, Responsibilities, and Competencies. May not include all duties of this job)
  • Consult and lead the Geriatric Transitions Program staff.
  • Provide training, mentoring, and assistance to new employees.
  • As requested by the program coordinator, perform other duties.
  • Work as a member of a multidisciplinary team.
  • Assess client need, risk factors and barriers to community placement at referral. Identify and record baseline presentation of 'challenging behaviors' at referral and track progress during service delivery
  • Identify existing services and supports (informal and formal) involved with client at time of referral. Seek to identify most appropriate role and input to compliment and extend existing service provision in the interest of facilitating a successful community placement.
  • Work in conjunction with existing mental health providers when present - seek to clarify and agree complimentary role to meet clients' needs and facilitate successful community placement.
  • Coordinate and facilitate care conference meetings to problem solve and address issues relating to challenging behaviors and community placement
  • Create effective client specific positive behavior support plans / Individual support plans to be utilized by clients and residential providers in community settings. Monitor use of the plans and provide trainings and consultation as needed to ensure effective use of the plan. Update and modify plans as needed.
  • Provide 1-1/group training as needed to residential providers, family members or other professionals involved in a clients' care, in the interest of effective implementation of positive behavior supports.
  • Provide crisis intervention and consultation by phone as needed.
  • Respond to out-of-hours requests for crisis intervention and consultation as needed.
  • Coordinate services with community agencies such as DSHS, doctors, hospital staff, discharge planners, other MH Providers, courts, Day Services, Home Care agencies, Employment Support agencies, residential providers etc.
  • Seek consultation and feedback from clients, family members, agency staff and residential providers
  • Record, track and monitor presentation of challenging behaviors at referral, placement, 30, 60 and 90 days as needed
  • Provide services and treatment in accordance with the evidence-based practice recovery model and document the same.
  • In coordination with community agencies and professionals, provide mental health services as appropriate including: direct clinical services such as intake, assessment, diagnosis, treatment, and crisis intervention; consultation/liaison with government and community agencies, hospitals, and other programs; community education, development, coordination and prevention/intervention programs; and program development and evaluation.
  • Provide clinical assessment and treatment to clients as specified in each client's individual treatment plan. Services may include but are not limited to: evidence-based practices, treatment planning, individual and/or group therapy including DBT and cognitive/behavioral, and crisis planning and stabilization.
  • Consult existing clinical staff and medication management services.
  • Provide timely and accurate compliance with all required and appropriate charting and coding as required by regulatory organizations such as Medicare, NSMHA, etc.
  • Comply with involuntary treatment admission (ITA) requirements as appropriate and/or coordinate least restrictive order (LRO) services if needed. Notify CDMHPs, write affidavits, and serve as a witness in court when involuntary treatment is indicated if needed - establish and clarify involvement if an existing MH Provider is also working with client. Continue to testify on client's behalf for the duration that Involuntary Treatment is indicated (at the end of 72 hours, 14 days, 90 days, etc.) if needed/appropriate.
  • Monitor compliance with LRO (Least Restrictive Order) as needed.
  • Deliver services as stated.
  • Seek continuance when indicated.
  • Complete all required documentation in a timely and thorough manner consistent with approved practices, policies and procedures.
  • Attend continuing education training as required by state regulations.
  • Attend all training required by Sunrise.
  • Maintain all on-going licensing and certification requirements for this position and supply Sunrise proof of the same.
  • Monitors illness management and resource needs of assigned clients.
  • Assists and advocates for the client in the following areas as needed (working in partnership with existing providers where present and identifying and clarifying establishing role expectations to compliment and extend existing services):
  • Housing: Assist and refer client to residential facilities, cluster houses and for section 8 vouchers when indicated, helps client apply for low-income housing when applicable, and follows through to assure housing is secured. Liaises between client and landlord as necessary in the process of securing and maintaining housing.
  • Basic needs: Connect client, including but not limited to food banks, meals on wheels, emergency shelters, in home support services and any community programs that will help provide food, clothing and/or shelter.
  • Crisis needs: Monitor client behavior to predict and avoid crises. Notify appropriate professionals, team members and DCRs, of client status, coordinate with service providers (hospitals, evaluation and treatment center, and crisis beds) to ensure client safety and monitor relapse prevention activities and strategies.
  • Employment: Refer eligible clients to vocational, pre-vocational enhancement programs (clubs, supported employment), or training programs.
  • Ethnic/Cultural support: Assist client as needed to connect to these supports and seek required and/or appropriate consultation for minority populations.
  • Financial needs: Inform, direct and assist clients in obtaining eligibility, entitlement and insurance benefits, connect and follow-up with Representative Payee if any, and direct client to budgeting assistance as needed.
  • Physical health care needs: Refer client to physician to address physical care needs, assist client in attending appointments. Assist the client to inform the prescriber for psychiatric med management services of their physical health care plan.
  • Psychiatric services: Help schedule psychiatric services as indicated, follows prescriber's directed treatment, monitors medication compliance and provides psychiatrist with ongoing information (as indicated) regarding client status.
Treatment Services
  • Utilize solution-focused and cognitive behavioral therapy and group process, provide this treatment, and share this knowledge with other staff.
  • Provide harm minimization interventions as appropriate
  • Communicate with consumers and family members to obtain information regarding consumer's needs and objectives, interests, skills, and support systems.
  • Develop consumer-focused, goal-oriented treatment plans, which address the specific needs of the consumer. Consult with existing MH providers when present to compliment and extend existing treatment plans.
  • Evaluate treatment effectiveness and revise care plans regularly.
  • Maintain consumer records in keeping with agency, county and state guidelines.
  • Act as a backup for other team members.
  • Respond promptly to client needs.
  • Support and encourage recovery model.
  • Utilize clinical supervision for clarification of questions and concerns, support, understanding role expectations, feedback and accountability.
  • Develop expertise in solution focused and cognitive behavioral therapy and group process, provide this treatment, and share this knowledge with other staff. • Provide quality oversight of documentation and assist in billing.
  • In addition to the above, any other responsibilities appropriate to the position and not specifically listed in this job description or tasks that may be assigned on an as needed basis.
Essential Requirements: (Any Equivalent Combination of Knowledge, Skills, Abilities,

Education, and Experience)
  • Education: Master's Degree or further advanced degree in counseling or one of the social sciences OR Psychiatric Nurse
  • Experience: At least two years of experience in supervised direct treatment of persons with mental illness and/or developmental disabilities, or a Mental Health Professional (MHP), or a professional with significant career related experience working towards hours of completion for MHP status.
  • Licensure/Certification: Mental Health Professional (MHP), or a professional with significant career related experience working towards hours of completion for MHP status.
    • Agency Affiliated Counselor (Licensed).
  • Other Considerations:
    • DSHS background clearance required.
    • Current valid Washington State driver's license, acceptable driving record, and proof of insurance.
    • Skill using Microsoft Office products including Word and Outlook. o Strong verbal and written communication skills.
    • Flexible. Demonstrated ability to exercise sound judgment in challenging dynamic situations including behavioral issues.
    • Skill in reading, synthesizing, and acting upon multifaceted information.
      • Ability to write effective positive behavior support plans specific to client need
      • Ability to train and coach caregivers in the implementation of positive behavior support and de-escalation.
      • Extensive knowledge of clinical issues pertaining to adult client populations, along with extensive understanding of the clinical process and intervention strategies used to address these issues.
      • Assess client's presenting problems, formulate individualized treatment plans, apply appropriate treatment modalities, and assess, monitor, and document client's response to clinical interventions.
  • Relate effectively with clients, agency staff, other community programs, and the general public.
  • Work across agencies and disciplines, and as a part of a multidisciplinary therapeutic team.
  • Adhere to professional and ethical standards of practice.
  • Demonstrate respect for diversity and a commitment to developing multicultural competency and sensitivity.
  • Extensive knowledge of protocols of NSMHA.
Schedule:
  • M - F
  • 8am - 5pm
Compensation:

• $26-$36.52/hour

• Wages are determined by applicable industry experience, education, and certifications/licensure.
