MDS Coordinator

The MDS Coordinator will be responsible for monitoring patient charts for correct coding, electronically transmitting data in a timely manner and communicating with the Interdisciplinary Team to create and carry out patient treatment plans.

- Schedule the ARD's for payment assessments with the Rehab Director daily, weekly and as needed in a manner that accurately captures the RUG category.
- Provide a schedule of ARD's and assessment types weekly and as needed to the IDT in order to facilitate the timely completion of MDS sections and CAA's by each discipline.
- Communicate to members of the IDT as needed based on timelines of completion. Expected to report any issues with timely completion to the Administrator immediately.
- Participate in the completion of and updating the patients' care plan.
- STAR mentality (Selfless, Tough, Accountable, Respectful).
- At least two years' experience in a skilled nursing, acute or post-acute setting.
- At least two years' experience in the completion of the RAI process and implementation of Medicare regulations and guidelines.
- At least two years supervisory experience as a unit manager, ADON or DON.
- Must be licensed in the state of Nebraska or compact state as a Registered Nurse.
- RAC-CT certification preferred.
