Description

GENERAL DESCRIPTION:

Performs nursing practices within the scope of the Nurse Practice Act of the State of Ohio. Performs daily duties in accordance with job standards and as directed by Director of Nursing. Follows prescribed procedures and policies of the facility and Federal, State, and local authorities, to ensure that quality nursing services are provided at all times.

MAJOR DUTIES AND RESPONSIBILITIES:

1. Protects and maintains confidentiality of residents, information and resident's rights.

2. Respects and shows kindness to all residents. Supports person centered care concept.

3. Maintains the standards of nursing care and implements the policies and procedures of the facilities nursing department, including Skin Grids, treatment plans, preventative skin care plans, fall prevention care plans, incident reports and follow through prevention plans.

4. Maintains current knowledge and skills with respect to facility activities, nursing procedures, and technology.

5. Attends team care conferences as needed.

6. Provides oral instructions to nursing assistants.

7. Records all pertinent information in resident's records, both written and EMR's.

8. Performs head to toe observation of residents and documents findings.

9. Provides resident's care and activities in accordance with their prescribed medical and nursing treatment plans.

10. Administers medications according to facility policies and procedures.

11. Maintains current CPR skills.

12. Functions as team leader and supervisor.

13. Supplies and interprets information, supports decisions, and facilitates communication with residents and their families, nurse aides and support staff.

14. Assures that residents call lights and alarm sounds are answered promptly.

15. Reports changes in resident's health status to physicians, NPs and other clinicians.

16. Receives and records oral telephone medication and treatment orders from physicians/physician's offices.

17. Report changes in resident's health status to physician, resident and/or responsible party.

18. Protects and maintains all work areas in accordance with facility safety policies.

19. Assures that floors, resident rooms, common areas and service rooms are free of hazardous debris.

20. Performs daily tasks in accordance with infection control and standard precaution policies and procedures.

21. Follows policies and procedures related to body mechanics and lift systems.

22. Dresses in accordance with facility policy.

23. Assures residents individual care plans are being followed by nursing assistants, LPN's reviewing the assignment sheets.

24. Assures that nursing assistants are completing their required documentation before the end of the shiftBincluding EMR's, Restorative Care, Food/Intake Records and all other records.

25. Assures rounds are done with shift to shift change to prevent conflict between shifts and ensure good continuity of care.

26. Maintain an adequate understanding of the QA/PI process and participate in performance improvement committees as requested.

27. Agree not to disclose assigned user ID code and password for accessing

resident/facility information and promptly report suspected or known

violations of such disclosure to the Administrator.

28. Agree not to disclose resident's protected health information and promptly

report suspected or known violations of such disclosure to the Administrator.

29. Report any known or suspected unauthorized attempt to access facility's

information system.

30. Assist the Director in preparing performance evaluations as directed.

ANCILLARY RESPONSIBILITIES:

1. Resupplies and maintains order of medication and treatment carts.

2. Is flexible to changes in schedule and works overtime as needed.

3. Participates on one or more committees of the facility including QA/PI.

4. Occasionally attends activities in the community with a resident.

5. Occasionally transports lab specimens to the hospital lab.

ASSIGNMENT, REVIEW, AND APPROVAL OF WORK: 1. Receives direction from Director of Nursing, ADON, and Registered Nurse.

2. Shows initiative and judgment in doing assigned tasks.

3. Ability to perform, organize, and evaluate work.

4. Work may be reviewed by Director of Nursing/ADON/RN or supervisor during or upon completion of shift.

RESPONSIBILITY AND DECISION-MAKING AUTHORITY:

Decision making is limited to specific responsibilities as assigned by Director of Nursing/ADON/RN or supervisor.

REPORTING PREPARATION:

1. Recorded, oral and written report as prescribed in policies.

2. All changes in resident's condition.

3. All hazardous conditions in the facility, including equipment.

4. All accidents and/or incidents observed.

EDUCATION & EXPERIENCE:
  1. Maintains current licensure from the State of Ohio.

SPECIFICATIONS - MENTAL/PHYSICAL:

1. Lifts routinely, objects weighing up to 100 lbs.

2. Stands and walks on hard surfaces 6-8 hrs. daily.

3. Bends, stoops, and turns often.

4. Pushes and pulls medication and treatment carts weighing up to 150 lbs. on a regular basis.

5. Pushes wheelchairs and recliners with residents on them, weighing up to 250 lbs.

6. Assists weight-bearing residents weighing up to 200 lbs. from bed to bedside commode, and to chair on a daily basis.

7. Exerts considerable force in pulling and pushing daily.

8. Hearing ability must be sharp to understand residents and their needs.

9. Visual ability must be sharp to understand residents and their needs.

10. Able to think clearly, and react calmly during emergency situations, and assist with possible evacuation of residents.

11. Work/tasks are performed mostly indoors.

12. Escorts residents outdoors for on/off ground activities.

13. Outdoor activity areas are well-lighted and clean, with some exposure to dust, dirt, and organic substances.

14. Exposure to heat, air conditioning, and humidity as controlled by the facilities' central system, with variations in temperature of 4 degrees on either side of the normal 76 degrees.

15. There are constant exposures to blood, body tissues and fluids, with occasional exposure to hazardous materials and infectious diseases.

16. Adjusts to exposure of loud and unpleasant voices.

17. Exposure to constant stress of working with sick residents and their families, regulatory compliance team.

18. Exposure to residents who are confused, irrational, agitated, and combative with abrupt mood swings, on a regular basis.

RELATIONS WITH OTHERS:
  1. Maintain cordial relations with residents, their families, physicians, contracted service providers, etc.
  2. Communicates clearly with staff and maintains cooperative spirit
LEARNING PERIOD REQUIRED:

1. Six to ten days and then on going. Length of orientation is determined according to the Nursing Staff Orientation Policy and Procedure.
