The Community Liaison responsible for direct customer support, procurement of home health signed physician orders and signed plan of care to include other required documentation. Assist home health medical records and clinical team managers in preparing the physician orders/plan of care/F2F for delivery to physician office. Maintain professional and ethical manner and collaborates with other members of the health care team to ensure quality patient care and excellent customer service. Willing and able to travel to other branches to conduct offsite training, attend outside meetings with physician offices, and maintain relationships to ensure best quality of care for our patients. Operates company vehicles safely and adheres to the responsibilities outlined in the Motor Vehicle Policy and Vehicle Fleet Safety Policy.

Primary Responsibilities:

  • Prepare physician orders/485s/Face to Face (F2F) for physician offices

  • Assists with checking in the signed documents once returned from the physician office

  • Assists with follow-up on physician orders/plans of care not returned within 30 days

  • Review and prepare spreadsheet of outstanding documentation for each branch with required information for Community Liaison Manager, Director of Operation of home health and Medical directors

  • Daily route logs must be accurate and fully filled out and turned in daily to direct supervisor

  • Daily vehicle inspection must be performed daily and recorded on route sheet

  • Ensure that a monthly vehicle inspection is completed with a supervisor at branch level

  • Responsible for setting up vehicle maintenance/oil changes and reporting any vehicle concerns to direct supervisor and Fleet director at Wellmed

  • Delivery and pick up of physician orders and plan of care’s to be signed by physician daily

  • Responsible for tracking missing documentation and following up to ensure timely receipt according to the applicable laws, federal, and state rules and regulations and Agency’s policy

  • Assist physician office pertaining to accuracy of documentation being delivered and work with home health clinical team managers for any corrections needed on the documentation-needing signature by physician

  • Assist physicians with information in reference to electronic signature in current EMR

  • Receives and addresses customer complaints by routing specific or complex questions to appropriate managers

  • Exhibits professionalism and is courteous with all customers and co-workers

  • Demonstrates knowledge and observance Notice of Privacy Practices

  • Participates in educational programs and all required in-service programs, as assigned by supervisor

  • Requires obtaining pre-approval from direct supervisor for overtime

  • Acknowledges and understands that each driver is to operate company vehicles safely and adhere to the responsibilities outlined in the Motor Vehicle Policy

  • Requires basic knowledge with current home health EMR, EPRG, TruCare

  • Responsible for checking email, HALO’s, team messaging system and voicemail to respond to time-sensitive messages from customers, leadership, lower-level management

  • Adheres to the time reporting policy and attendance policy per UHG

  • Performs additional duties as assigned by manager

Required Qualifications:

  • High School Diploma/GED (or higher)

  • 1+ years of experience in medical physician office or home health back office administrative duties

  • Intermediate level of proficiency in computer skills and knowledge of Microsoft Word/Excel processing software

Preferred Qualifications:

  • Knowledge of general office procedures

  • Marketing experience

Soft Skills:

  • Attends all required in-service training programs to maintain comprehensive home healthcare knowledge base

  • Demonstrates effective time management skills by completing assignments within projected timeframes

  • Expresses verbal and written communication in a clear, positive, and cooperative manner

  • Demonstrates personal responsibility with regard to attendance and punctuality

  • Promotes Agency philosophy, mission statement, and administrative policies

  • Maintains privacy and confidentiality with regard to all patients, their families, staff, and Agency information

  • Promotes the Agency image by adhering to the Dress Code Policy

  • Follows the appropriate chain of command in all forms of communication

  • Demonstrates flexibility, enthusiasm, and willingness to cooperate while working with others or in place of others as necessary
