Case Mgmt Assistant- ASA VII
The Position Assists Case Management and Social Work staff with setting up patients’ discharge needs.
Hourly Range: $18.50 - $23.78
Principal Responsibilities
-
Assists with obtaining Durable Medical Equipment (DME), including but not limited to rolling walkers, commodes, wheelchairs, quad canes and hospital beds.

Make basic transportation arrangements (MAS, Rehab, back transfer, cabs) once discharge planner has documented need.

Maintains patient confidentiality of all communications and documents as required by hospital policy.

Assists with fax attaching and print attaching information into Allscripts.

Rounds frequently on assigned units.

Keeps Case Managers and Social Workers apprised of status of discharge plans, documents the status of activities and communications, and maintains adequate reporting information in Allscripts.

Maintains daily documentation in Allscripts of all activities done utilizing interventions.

Enters data into Allscripts in a timely fashion to facilitate discharge plans.

Maintains skills needed for performance of duties.

Utilizes available educational opportunities for improving skills.

Attends required in house in-services.

Projects positive image of the department and hospital.

Qualifications and Ideal Characteristics

HS diploma or equivalent and 3-5 years’ experience.

Proficient with computer/software applications.

Strong written and verbal communication skills and organizational skills.

Must be able to multi-task and prioritize.

While performingthe duties of this job, the employee may be subject to infectious materials and chemicals(see unit specific MSDS information).The noise level in the work environment is usually moderate.This job requires as an essential function that the majority of the time the employee bephysically on-site as the work cannot be done from a remote location.
