Summary

We are seeking a Nursing Care Specialist to join our team and take on the primary responsibility for completing the admission process for our patients. This includes assessing, planning, implementing, and evaluating patient care plans. As a Nursing Care Specialist, you will also be responsible for coordinating communication with families, nursing home staff, and other healthcare professionals involved in the care process. In addition, you will be expected to complete initial plan of care in the electronic medical record system, perform head-to-toe assessments, and ensure that all appropriate treatments and orders are in place.

Job Qualifications

  • Graduate of accredited school of nursing
  • Bachelor's and/or Master's degree preferred
  • Current nursing license in good standing
  • Minimum two years of varied work experience as a professional nurse, with one year preferably in a post-acute setting
  • Demonstrated skill in administrative performance, including written and oral communications and documentation
  • Ability to assess and respond to the needs of patients and families in varied settings
  • Capability to cope with family emotional stress and tolerance of individual lifestyles
  • Ability to lift, transfer, and/or move items or patients with weights of 10 - 200+ lbs using proper lifting techniques
  • Strong critical thinking and psychosocial assessment skills
  • Manual dexterity and skillful use of hands when working with patients and equipment
