The Mission Control RN is responsible for providing high-quality patient care to various patient populations within the framework of the YNHHS Home Hospital Model and includes the following key elements:
- Utilizing monitoring, assessment, planning, intervention and critical thinking skills to care for acute care patient populations in the home
- Navigating patients and families across the continuum of the YNHHS Home Hospital phases of care
- Monitoring and responding to biometric data per YNHHS Home Hospital protocols
- Conducting home and audio/visual patient care visits as directed by the care team and/or protocol
- Ensuring all interventions in the patient’s care plan are implemented efficiently with a patient centered approach
- Leveraging the resources and expertise of the YNHHS Home Hospital team and supplier partners to implement collaborative workflows, standards, policies, protocols, guidelines and documentation systems to support safe, reliable, high-quality, evidence-based care with clinical protocols as the foundation
- Adhering to the clinical protocols and be willing to obtain the necessary training to provide care within the context of the providing evidence-based care
- Promoting professional practice and a culture of safety; willingness to engage in process improvement efforts
Clinical Roles and Responsibilities:
- Providing protocol driven assessment, care coordination and interventions as appropriate informed by YNHHS Home Hospital protocols for diverse patient populations with a focus on oncology, medicine and post- operative care
- Exhibiting sound clinical decision making and critical thinking skills
- Maintain and practice in accordance with nursing guidelines per state and hospital regulations
- Facilitating admissions to YNHHS Home Hospital utilizing our guidelines
- Serving as a role model for professional nursing practice
- Utilizing nursing knowledge to identify, prevent and/or solve complex acute and/or recurring patient care problems
- Sharing expertise with the multidisciplinary team and serve as a resource to staff in area(s) of expertise
- Supporting research efforts to advance knowledge and promote evidence-based practice
- Preforming other duties and responsibilities as assigned by the Director of Nursing
Quality and Safety Responsibilities:
- Ensure compliance with reporting requirements in RL Solutions for Sentinel Events and Serious Incidents
- Participate in quality and safety investigations and root cause analyses
- Take appropriate action to ensure patient safety as a result of any investigation; and
- Serve as a resource in quality improvement processes and technique
Qualifications
EDUCATION
Graduation from an accredited School of Nursing. All newly hired Registered Nurses must have current BCLS certification or must obtain within one month of hire.
EXPERIENCE
Registered Nurse is the foundational job description for all RNs. All RNs are encouraged to advance through the SPIRE clinical advancement program.
LICENSURE
Licensed and a currently registered nurse in the State of Connecticut. Clinical specialty certification highly encouraged.
PHYSICAL DEMAND
Requires prolonged standing; walking; use of sight, hearing, and touch. May be exposed to infectious diseases. Will be exposed to physical, mental and emotional illness as well as end of life conditions and death. Moderate stress and risk of injury from patient care. Standing/walking for more than half of the day. Frequently bending, reaching, pushing, pulling, twisting and lifting. Must be able to assist pushing/pulling/lifting patients ranging from five (5) to three hundred (300) pounds.
