Social Services Coordinator

Our company is looking for a Wound Care Nurse (RN) to join our team.

Responsibilities:
  • Complete MDS documentation for Skin Condition (Section M) for all facility residents
  • Evaluates the effectiveness of nursing and other clinical education programs
  • Attend staff meetings, in-service classes, and committee meetings
  • Participate in weekly wound team rounds
  • Establishes competencies for clinicians regarding wound care
  • Collaborates with other members of the Education Department on joint educational initiatives
  • Collaborate amongst Interdisciplinary Team to encompass all aspects of care to promote wound healing
  • Assist in the implementation of individualized treatment plans for each assigned resident
  • Participate in the development and achievement of Wound Care Program and objectives
  • Perform all other duties as requested
  • Communicate in a positive, caring, and helpful manner with residents, family members, and other staff
  • Develops educational modules for wound care to be used for internal and external education, including teaching tools for patients and families
  • Respect the dignity of each person
Requirements:
  • Ability to lift 50 pounds occasionally, 20 pounds frequently and 10 pounds constantly
  • Ability to work and stand 80% of an 8-to-12-hour shift
  • Certified BLS or completion in first 90 days of employment required
  • Excellent presentation skills and organizational skills required
  • WCC certification, CWCN, CWOCN or CWS is preferred or certification pending within one year
  • Excellent time management skills
  • Ability to perform effectively in fast-paced, stressful environment
  • Excellent physical assessment and critical thinking skills
  • Excellent communication skills
  • Confers with the Director of Nursing and/or other licensed nursing personnel regarding dermatologic disorders of residents in the facility
    • Valid State RN License required. Previous geriatric nursing experience preferred
    • Current RN license required
