Physical Therapist provides rehabilitation and residential services for individuals with CARF accredited program Traumatic Brain Injury (TBI), Acquired Brain Injury (ABI), Spinal Cord Injuries, and Neurological Disorders.

Summary: The Physical Therapist is responsible for helping persons served to improve their physical function, mobility, and quality of life.

Primary Responsibilities
  • Evaluations to diagnose a person's physical limitations to develop a comprehensive understanding of their conditions.
  • Develops individualized treatment plans that outline goals, interventions and expected outcomes.
  • Educates persons served, families and staff about their conditions, treatment options and strategies for self-management and prevention.
  • Treat and document progress, modifying treatment plans as necessary to ensure optimal outcomes.
  • Attend person served team meetings as needed.
  • Maintain detailed and accurate records in compliance with state and federal regulations using EMR system.
  • Other duties as assigned.
Qualifications
  • Licensed Physical Therapist in the State of Michigan
  • Strong communication, interpersonal and problem-solving skills
  • Attention to detail and have the ability to prioritize independently
  • Intermediate knowledge of MS Office
  • Ability to perform physical tasks required for patient care
  • Empathy and genuine desire to help persons served improve their quality of life
