Overview

Halifax Health is seeking a Community Action Team Therapeutic Mentor for Halifax Behavioral Services.

Summary

The primary focus is to assist clients with functioning in the least restrictive environment. The mentor is responsible for linking, monitoring, and advocating for all assigned clients. The mentor assures that all assigned clients received the most appropriate services available to meet their identified needs.

Job Qualifications

Certification as a certified recovery peer specialist certified by the Florida Certification Board; or Associate degree in Human Services related field with one (1) year experience with children and adolescents in a clinical or recreational setting. Ability to read, analyze, and coordinate with multidisciplinary teams regarding procedures, and governmental regulations. Ability to draft reports and document mentoring services and effectively communicate verbally with, provide information to, and respond to questions from managers, patients, service providers, managed care entities, family members, and other stakeholders. Valid Florida driver's license with a safe driving record.

SKILLS, EXPERIENCE AND LICENSURE:

Knowledge and understanding of the characteristics of children, adolescents and adults with mental health issues, their families, and family systems.  Knowledge of community resources or ability to learn quickly and good organizational, time management and communication skills.  The ability to work well with community partners and in a team environment is essential. Highly motivated, self-directed, willing to work a variety of hours, including evening and on-call. 

Job Duties and Responsibilities

DUTIES AND RESPONSIBILTIES:

Provide individual, family and group coaching. Provide supportive counseling and crisis intervention as needed. Monitors individual client progress regularly. Complete all necessary documentation and maintain records. Adheres to agency policies and procedures, safety guidelines, business ethics and dress code. Communicate effectively orally and written. Performs all other duties as assigned and all required training. Provide respite services (short term supervision of individual away from family) and non-traditional case for management services. Provide on-call crisis intervention services during nighttime and weekend hours as assigned. Assist with transportation to medical appointments, court hearings and other related activities outline in plan.

PHYSICAL DEMANDS:

Walking, standing, or sitting for extended periods of time resulting in moderate fatigue.

WORKING CONDITIONS:

Pleasant inside environment. Travel and home visits are done routinely. Some evening, night and work required.
