Requirements:
- VT License
- Minimum of 3 years prior community hospice experience
- BCLS
- Hospice/End of Life-Complex symptom management and education, Symptom assessment and management with high level of critical thinking skills.
- Comfort initiating and participating in goals of care discussions and planning with patients, family, and team
- Medication management and administration (IV, Subcutaneous, oral, sublingual, rectal, and other routes
- Facility will accept COVID and FLU declinations
