Purpose: Provide oversight and coordination to MCHWC's grant activities. Understands and identifies gaps in organizational and program funding and actively collaborates with counterparts in Grant Office to search for new grant opportunities to address these gaps. Maintain relationships with program directors, managers, external grant managers and program officers of funding bodies.

  1. Grants Management and Oversight


  • Develop and maintain a comprehensive understanding of MCHWC's current grant funding and the associated programs which are funded via those dollars.
  • Maintain a succinct summary document/grant matrix that provides an overview of MCHWC's current grant portfolio including amount of award, grant year, contract amount, grant termination date, anticipated date of continued funding application (CFA), etc.
  • Monitor grant-funded program goals and objectives, work plans, and budgets on an ongoing basis to ensure targets are met.
  • Ensure accurate and timely preparation of grant reports, as well as, vouchering in conjunction with Finance department and program staff. Develop processes to ensure that program and financial reporting support one another.
  • Communicate grant information and proposal and reporting deadlines to relevant agency departments and all appropriate internal users.
  • Prepare written reports for key stakeholders, including Executive Leadership, Board of Directors and other external audiences.
  • Assist with the development and management of budgets and subcontracts.
  1. Continued Funding Applications (CFAs)
  • Coordinate proposal development with program staff (care management, community health initiatives, clinic, development, etc.).
  • Assist in the editing and submission of CFAs, with a focus on federal and state funded grants.
  • Work with Finance Department and program leadership to develop grant budgets that are reflective of narrative proposal.
  • Review and approve CFAs and shepherds them through internal approval process.
  1. New Grant Opportunities-Identification and Evaluation
  • Identification and Evaluation
  • In collaboration with the Director, Grants Management, monitor and respond to new funding opportunities aligned with the agency's strategic priorities.
  • Researches potential private and government grant opportunities by monitoring: listservs, Grants.gov, California State grants, searchable foundation databases, and other organizations' grants announcements, websites, and newsletters.
  • Conduct initial outreach to funder to obtain additional information about their funding opportunities and to determine if a specific organizational project would be considered a compelling case for funding.
  • Maintain a record of contacts, related information, upcoming opportunities, and reoccurring opportunities.
  • Assist in the writing and editing of grant applications.
  1. Proposal Development, Preparation, and Submission
  • Create specific project plan (with tasks, assignments, and timelines) to ensure completion and submission of proposal within required time frame and in compliance with requirements.
  • Assemble and review all proposal components for completeness, consistency, and continuity amongst sections. Edit submissions as appropriate and requests additional information and details as required.
  • Work with finance department, human resources and program leadership to develop grant budgets and to ensure continuity between written proposal and finance documents.
  • Follow organizational process for review and sign off completed proposal.
  • Submit all grant application components in compliance with funder's requirements.
  • Archive all proposal components in a secure, but readily accessible, electronic file.
  1. Project Management
  • Manage multidisciplinary workgroups during the completion of CFAs, site visits, agency response plans, or other related projects.
  • Manage and provide support to the organization's long-term initiatives and short-term projects utilizing the 5 key process groups of project management: Initiate, Plan, Execute, Monitor and Control, and Close.
  • Consult with and advises project leaders (i.e.: Executive Champion and Project Sponsor) regarding effectively managing project scope, schedules, budgets, and changes.
  • Ensure project-reporting requirements are met.
  • Secure needed training in collaboration with Human Resources and department supervisor to teams to achieve performance targets.
  • Build and maintain strong internal and external stakeholder relationships through regular meetings, follow up, and communications.
  • Provide support/assistance as assigned for organizational wide initiatives and projects.
  1. FQHC Designation Support
  • Support FQHC Administrator in the management of all FQHC related activities and required tasks during Operational Site Visit preparation.

Qualifications

Bachelor's degree required. Proven track record securing grant funding and program management including, but not limited to, local and state government grants, federal funding requests, local grants, capacity building proposals, and funding requests to private and family foundations. Requires a minimum of three to four years' experience in grant writing, administration and project management, as well as a direct program provision and/or management. Requires demonstrated working knowledge of grants and grant administration systems, processes, and budgeting. Ability to comprehend and make inferences from written material and systematically compile information to prepare reports; review the work products of others to ensure conformance to standards; communicate with strong oral and written skills and a proven ability to effectively interface and work with individuals within and outside of the organization; work effectively and collaborate with a variety of staff at all levels in the organization, governmental representatives, and foundation program officers. Must be highly proficient in Word and Excel.

Equipment/Machinery: Fax machine, copier, personal computer, telephone, calculator, Microsoft Word/Excel, EHR system, and other software as required.
