Position Overview: Physical Therapist (PT)

As a Physical Therapist with Pinnacle Home Care, you will be at the forefront of providing exceptional care to patients in their own homes. You will work alongside a physician and nursing supervisor to ensure compliance with all clinical practice and regulatory standards.

Key Responsibilities

  • Conduct initial and ongoing patient assessments, including OASIS documentation.
  • Create and implement customized treatment plans in collaboration with physicians, making adjustments as necessary.
  • Collaborate with the interdisciplinary care team to achieve optimal patient outcomes.
  • Educate and support patients and their families on their therapy programs.
  • Supervise Physical Therapist Assistants on a regular basis every 30 days.
  • Communicate patient progress and any necessary changes to physicians and care teams.
  • Document all visits and progress notes on the same day of service.

Qualifications

  • Graduation from an accredited Physical Therapy program.
  • Active Physical Therapist license.
  • Current BLS certification.
  • Valid driver's license and access to a reliable, insured vehicle.
  • A minimum of one year of PT experience; home health experience is preferred.
  • Proficiency in using electronic medical records is a plus.
  • Physical ability to perform necessary tasks including standing, bending, and lifting.
  • Excellent self-management, communication, and judgment skills.
