Direct Supervisor: Clinical Supervisor

Job Purpose Summary:

  • Intake, assessment of needs, service planning, service plan implementation, service coordination, monitoring and follow-up, reassessment, case conferencing, crisis intervention, and case closure.
Responsibilities Include:
  • General Case Management functions supporting client needs.
  • Collaborating with Clinical Supervisor on important collateral needs.
  • Attend daily huddle meeting to coordinate daily case management needs
  • Coordinate/facilitate Stress Management and Vocational Groups.
  • Participate in weekly Treatment Team Meetings.
  • Assist clients with securing resources.
  • Back-up for completion of Resource Assessment
  • Discharge Aftercare planning.
  • Maintain communication with families, patients, and staff.
  • Assist clients and families with general billing questions.
  • Documentation/Record keeping dialogue.
  • Back-up Front Desk Receptionist.

Job Type: Full-time

Required education: Associates Degree or college coursework related to this position, Bachelor's Degree Preferred.

Required experience: 2-4 years' experience working in a Drug & Alcohol Treatment Facility.

Proper Knowledge and Skills:
  • Proper English and grammar, usage and spelling
  • Software application related to the field
  • General offices principles and practices
  • Basic computer knowledge including email etiquette
  • Professional phone and customer service demeanor
Physical requirements:
  • Must be able to lift 20-50 lbs.
  • Occasional to frequent walking, sitting, standing, driving
  • Close eye work (computers, typing, reading, writing)
