Geriatric Medicine opening in Greensboro, NC _x000D x000D_ Full time, permanent opening_x000D_ Must be BC/BE Family Medicine, Internal Medicine, or Geriatric Medicine_x000D_ Provide seniors with affordable health care_x000D_ We are an outcomes-focused, value-based organization and for their panel of patients, the following metrics are regularly measured to help PCPs become successful and reach partnership status: patient admissions/thousand, using between 18-21 appointment slots per day (each new patient count for 2 slots, follow-up patients 1 slot), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures_x000D_ Each PCP will have goals for these metrics and will be expected to work towards those targets with their center and market leadership as well as their care teams_x000D_ Culture is very important and because PCPs are leaders in the organization and centers, they are expected to help champion a positive culture of love, accountability, and passion along with center leadership_x000D_ Competitive compensation and benefits package offered_x000D x000D_ Job Duties/Responsibilities: _x000D x000D_ Independently provides care for patients with acute and chronic illnesses encountered in the older adult patient_x000D_ Take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient_x000D_ Engage with the hospitalist whenever one of their patients is in the hospital_x000D_ Responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination, and documentation for patients with acute and complex chronic health needs._x000D_ Lead their care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office_x000D_ For patients that are unable to come to the office in the hospital, SNF, LTC, or homebound, PCP will engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market._x000D_ Have an active role in the management of their center and will help cover for other providers who may be out for various reasons. It is also expected that each PCP will take an active role as needed in recruiting patients for the center and additional providers for the company_x000D_ Performs other duties as assigned and modified at the manager's discretion
