Trinity Health Of New England is looking for a Crisis Clinician to join our team of mission-driven healthcare professionals located on our Emergency Room at Saint Francis Hospital and Medical Center.
The Crisis Clinician and Therapist i s responsible to serve as the driver of a collaborative, interdisciplinary process which coordinates, monitors, and plans behavioral health care services while patient is receiving care in the Emergency Room. The Crisis Clinician Provides a comprehensive psychosocial assessment of patient and family in order to develop treatment plan and goals and is an advocate for the patient to ensure optimal attainment of treatment goals
What you will do:

Performs assessments, provides support to patients and families and is a patient advocate to assure appropriate cares is received . Contributes to overall quality of care through meeting the individual needs of our patients in order to improve the quality of care, reduce costs of care, and enhance patient satisfaction

Effectively communicates with patients, families and the integrated clinical team to ensure everyone understands the needs of the patient to ensure a safe discharge plan is created.

Requirements :

Education: MSW degree from an accredited School of Social Work or Behavioral Health Field required.

CT licensure in appropriate professional designation preferred, i.e. Clinical Social Worker, Psychologist, Marriage and Family Therapist, or Professional Counselor.

Excellent written and verbal communication, knowledge of community resources, social work theory and practice.
