Job Overview

The Physical Therapist will complete screening, evaluations, re-evaluations, and discharges as supervised by the Physician. The Physical Therapist will provide supervision to Physical Therapy Assistants as directed by the Director of Rehabilitation. Full-Time

Responsibilities and Duties

  • Evaluates patients assigned per MD orders and as indicated, and establishes an appropriate Plan of Care and Goals that meet the specific needs of the individual
  • Re-assesses patients and completes Updated Plan of Care at least every 30 days, or as indicated on initial evaluation, in order to determine the need for continued services or discontinuation of services based on patient progress. Carry out treatment plan as indicated
  • Screens all patients in facility on a quarterly basis, to identify those who have experienced declines in their functional abilities and determine if Physical Therapy Intervention is required
  • Provides thorough discharge planning that will allow the patient to function at a maximal level of independence in the least restrictive environment
  • Observes and records patient's progress, response to treatment and limitations and maintains this information in electronic medical record
  • Completes required documentation at point of service
  • Supervises Physical Therapy Assistants as assigned by the Director of Rehabilitation
  • Provides training and education to staff, family, and caregivers as indicated
  • Attends and reports accurately a patient's status at weekly UR meeting and interdisciplinary care plan meeting
  • Identifies needs for program development and implement programs according to company policies and procedures
  • Coordinates in-services for facility staff as need is determined, to ensure safe and effective carry over of Restorative Programs and overall call of the residents
  • Attends all facility required meetings
  • Takes a patient centered approach to provision of care
  • Maintains professionalism and follow all national/state associations regulations at all times
  • Attends continuing education courses specific to current position

Qualifications
  • Education: Graduate of an accredited physical therapy program
  • Licenses/Certifications: Current Physical Therapist state license
  • Maintain state and national licensure requirements
  • CPR certified
  • Aquatic PT experience preferred
  • Able to perform therapy based treatment in a pool
  • Knowledge of the principles and practices of treatment in an aquatic facility
  • Knowledge of optimal swimming pool water chemistry, temperature, and cleanliness standards
