The Intake Coordinator ensures seamless coordination between Amazing Care HHS, Inc. and referral sources. This role is responsible for accepting new clients and ensuring we can provide the necessary services. The Intake Coordinator works closely with the clinical team to allocate appropriate staff for new clients, adhering to Amazing Care's philosophy, standards, and policies.

Position Requirements:

  • Strong verbal and written communication skills.
  • Ability to multitask.
  • Experience with computers and Microsoft programs.
  • Ability to work independently with minimal assistance.

Responsible To: Client Services Manager

Responsibilities:

  • Maintain professional relationships with all referral sources.
  • Manage all aspects of the referral and staffing process.
  • Ensure client eligibility for services under their insurance plan.
  • Obtain signed and dated orders and ensure continuous home health orders from a following physician.

Performance Standards:

  • Maintain and evaluate incoming referrals to determine staffing capabilities for medical needs.
  • Provide clinical managers with referral information to ensure adequate care and correct field staff information.
  • Communicate referral information to field staff.
  • Contact new clients to verify details (address, phone number, PCP) and inform them about their assigned field staff.
  • Enter referral information into the EMR system to create patient profiles.
  • Understand the insurances we accept and the services they cover.
  • Verify insurance eligibility for all referrals.
  • Confirm any additional payers.
  • Develop and maintain good working relationships with case managers.
  • Communicate with referral sources to ensure adequate doctor's orders for all prescribed services.
  • Verify ongoing physician support for home health orders.
